𝗧𝗼𝗻𝗴𝘂𝗲 𝗧𝗵𝗿𝘂𝘀𝘁𝗶𝗻𝗴 & 𝗶𝘁𝘀 𝗜𝗻𝘁𝗲𝗿𝗰𝗲𝗽𝘁𝗶𝘃𝗲 𝗮𝗽𝗽𝗹𝗶𝗮𝗻𝗰𝗲𝘀.

1. Since the Max force of Tongue during swallowing is thrusted on Incisors, Tng tamers are placed on palatal surface of U/L incisors.

2. Lat Tng Thrust is the result of either Macroglossia or Narrow, constricted, V shaped U/L arches. We have spcl Lat Tng Cribs for its prevention.

3.Ask the pt to swallow normally & tell u where does his Tng rests during deglutition. If its Palatal rugae, its normal.

4. If the Tng rests behing U-incisors, give him U-Fxd Palatal cribs.

5. If the Tng rests on L-incsrs, provide him L-fxd Tng spikes.

6. Latest advancements of these cribs are in the form of Tongue Tamers, bonded palatally on U/L incisors.

7. For those Adult cases, where Tng training is required for Tng thrust, Word Stutter & Speech Therapy, etc, Blue Grass Appliance is used.

𝗪𝗵𝘆 𝗡𝗶𝗧𝗶 𝟭𝟵𝘅𝟮𝟱 𝗶𝘀 𝗡𝗢𝗧 𝘂𝘀𝗲𝗱 𝗳𝗼𝗿 𝗥𝗲𝘁𝗿𝗮𝗰𝘁𝗶𝗼𝗻???

1.Usually, SS 19x25 is an ideal wire for bodily movement of teeth during retraction.

2. Comprised of Cu-NiTi set of wires, even Damon uses SS 19x25 wire for translational movement.

3. This is because of its thick, stiff & heavy nature, smooth sliding mechanics, ability to maintain integrity of the arch better Anteroposterior correction & good vertical control.

4. Though NiTi 19x25 is thick, heavy & stiff, too but lacks the rigidity due to its springback nature. Besides, its good for A+L only, bcoz its shape memory behaviour goes against the retraction mechanics. Hence, its not used in extn space closure.

5. On excessive retraction forces, the resiliency of NiTi wires cause deepening of bite. Hence, it cannot be used in Deep bite & Steep COS cases.

6. Though enough evidence is not available, but NiTi wires show more friction in metal brackets, due to material difference,as compared to SS wires, during space closure.

5 yrs back, long before Myself using TADs regularly in my practice, I used to place a thick stiff rectangular wire in the FB hole of the molar tube bonded to U7 & Bite Turbos on L7. This main arch wire used to be secured by lig ties. This provides stability to post seg.

An Auxiliary NiTi 12 or 14 was used to pull the molar up same as in Piggy Back Technique.

SS 17x25 wire lig tied on 5&7 passing thru major FBhole of molar tube with its end cinched back. This gives stability to post seg.

Bite turbos on opp 5&7 to prevent extrusion of 5&7.

NiTi 12or14 passing thru wire hole of molar tube & sitting overlapped in the 5's brckt slot. It gradually pulls up the molar with increasing sequence in due course of treatment.

Depending upon the movement of molars, the Anchorage are classified as:-

𝟏. 𝗠𝗮𝘅𝗶𝗺𝘂𝗺 (𝗛𝗶𝗴𝗵) 𝗔𝗻𝗰𝗵𝗼𝗿𝗮𝗴𝗲= Those 𝗕𝗶𝗺𝗮𝘅 𝗰𝗮𝘀𝗲𝘀 𝘄𝗶𝘁𝗵 𝗘𝘅𝗰𝗲𝘀𝘀𝗶𝘃𝗲 𝗢𝗝 in which we want to utilize all the space available for retraction without loosing a single mm, are called as Max Anchorage.
𝗘𝘅:- 𝗡𝗮𝗻𝗰𝗲, 𝗧𝗣𝗔, 𝗧𝗣𝗡.

𝟮. 𝗠𝗼𝗱𝗲𝗿𝗮𝘁𝗲 (𝗠𝗲𝗱𝗶𝘂𝗺) 𝗔𝗻𝗰𝗵𝗼𝗿𝗮𝗴𝗲= Those 𝗦𝘂𝗯𝗱𝗶𝘃𝗶𝘀𝗶𝗼𝗻 𝗰𝗮𝘀𝗲𝘀 in which we want equal movement of Ant & post seg to close the extn spaces are called as Moderate Anchorage.
𝗘𝘅:- 𝗦𝘁𝗲𝗲𝗹 𝗹𝗶𝗴𝗮𝘁𝗶𝗼𝗻, 𝗼𝗺𝗲𝗴𝗮 𝗹𝗼𝗼𝗽𝘀, 𝗮𝗻𝗰𝗵𝗼𝗿 𝗯𝗲𝗻𝗱𝘀 𝗺𝗼𝗹𝗮𝗿 𝘀𝘁𝗼𝗽𝘀, 𝗰𝗿𝗶𝗺𝗽𝗮𝗯𝗹𝗲 𝘀𝘁𝗼𝗽𝘀, 𝗲𝘁𝗰.

𝟯. 𝗠𝗶𝗻𝗶𝗺𝘂𝗺 (𝗟𝗼𝘄) 𝗔𝗻𝗰𝗵𝗼𝗿𝗮𝗴𝗲= Those 𝗠𝗼𝗹𝗮𝗿 𝗠𝗲𝘀𝗶𝗮𝗹𝗶𝘀𝗮𝘁𝗶𝗼𝗻 cases where we want to close the major extn space by Post seg & only minor amt by Ant seg are called as Minimum anchorage.
𝗘𝘅:- 𝗟𝗮𝗰𝗲𝗯𝗮𝗰𝗸𝘀 & 𝗕𝗲𝗻𝗱𝗯𝗮𝗰𝗸𝘀 𝗼𝗿 𝗰𝗶𝗻𝗰𝗵𝗲 𝗯𝗮𝗰𝗸𝘀.

𝟰. 𝗔𝗯𝘀𝗼𝗹𝘂𝘁𝗲 𝗔𝗻𝗰𝗵𝗼𝗿𝗮𝗴𝗲= Those 𝗚𝘂𝗺𝗺𝘆 𝘀𝗺𝗶𝗹𝗲 & 𝗩𝗠𝗘 𝗰𝗮𝘀𝗲𝘀 where U want total retraction along with intrusion of the Ant seg without using post seg OR
those 𝗠𝗼𝗹𝗮𝗿 𝗠𝗲𝘀𝗶𝗮𝗹𝗻̃ /𝗗𝗶𝘀𝘁𝗮𝗹𝗻̃ 𝗰𝗮𝘀𝗲𝘀 where u want to mobilize the post seg without affecting Ant seg are called as Absolute Anchorage.
𝗘𝘅:- 𝗧𝗔𝗗𝘀, 𝗕𝗼𝗹𝗹𝗮𝗿𝗱 𝗔𝗻𝗰𝗵𝗼𝗿 𝗛𝗼𝗼𝗸𝘀, 𝗛𝘂𝗴𝗼 𝗺𝗶𝗻𝗶 𝗽𝗹𝗮𝘁𝗲𝘀, 𝗲𝘁𝗰.
