Restoring Smiles Safely: Telescopic Dentures for High-Risk Geriatric Patients

A female patient with multiple comorbidities presented with reduced masticatory efficiency due to multiple decayed and missing teeth. Given her compromised medical status, a minimally invasive dental intervention was indicated. Several decayed teeth underwent endodontic therapy and were decoronated at the gingival level to serve as abutments. Utilizing support from remaining healthy teeth, a telescopic overdenture was fabricated. Telescopic dentures offer superior retention, enhanced stability, and increased patient comfort by preserving alveolar bone and periodontal proprioception.

Hospital-Based Dental Extractions
An elderly patient with unstable angina requiring antiplatelet and anticoagulant therapy needed multiple fractured teeth extracted for preprosthetic mouth preparation. To protect her cardiac health, the patient was advised to continue her blood thinners. Due to her unstable systemic status, the procedure was performed in the hospital Intensive Care Unit (ICU) under the direct supervision of her cardiologist, with full emergency protocols in place. The extractions were minimally invasive and completed without complications.
