I had a patient who came in for a smile makeover with veneers as she was unhappy with her smile.
On examination, there was vertical maxillary excess with gummy smile.
We did a thorough scaling and I spoke to her about an esthetic crown lengthening.
Steps followed
1. Took a CBCT with lips retracted with cotton so you can appreciate the bone levels and know where the CEJ lies and if you need an osteotomy. In this case I needed an osteotomy.
2. Used a laser for gum.contouring. I find that with the blade the contour is not as I want it.
3. Raised a flap full thickness flap beyond MGJ
4. Removed bone using pre-made Stent, cbct and soft tissue removed as guidelines.
5. Suturing starting from palatal to buccal. Used only numbing gel on palatal.papillae. No need to raise palatal flap. It is much easier if u start suture and end on palatal.
