A stitch in time saves nine! (1/2)
A shy, sweet girl presented with a simple complaint- gap in the bunny teeth.
The abnormality, though was more than what met the eye.
On examination, she exhibited a Class II skeletal relationship with a retrognathic mandible (backwardly placed lower jaw) with a horizontal growth pattern with a Class II division 1 dental relationship. She had a deep mentolabial sulcus, deep nasolabial folds and a reduced lower facial height owing to the severe deep bite.
The beauty with treating growing kids is the possibility of harnessing active growth and redirecting it to correct jaw relationships- Eliminating the need for a possible jaw surgery as an adult. Isn’t that great? Or should I say almost magical? Timely intervention is key here- the bone responds faster in a younger age group.
Treatment plan consisted of a Removable Functional appliance- The Twin Block with the objective of advancing the mandible (lower jaw). The Visual Treatment Objective was positive- green lighting a Functional appliance as the most suitable treatment of choice.
Full credit to her- she was a very compliant patient. We were able to achieve excellent results in a short period of time- with a phenomenal increase in the lower facial height, significant bite opening, a flatter mentolabial sulcus and an advanced mandible.
6 months after the treatment, the mandible has truly stabilised in the advanced position, the growth re-directed successfully in our favour (that lower facial height) and our girl is also now a full fledged teenager all set to undergo her phase II treatment- Braces!
