Fantastic smile transformation for this young man who came in for picture perfect 32! (Part 1/2)

He came in with a lot of scepticism about his journey since he was studying outside India. He wanted something aesthetic and not very time consuming. We went ahead with clear aligners and viola!

The challenge was planning the visits and procedures limited to his duration of stay in Mumbai. No hassles of looking for an Orthodontist in a foreign country!
With the patient’s commendable compliance managing studies and hectic schedules, we were able to achieve the results with merely 4 visits in a span of 6 months!

A stitch in time saves nine! (2/2)

Phase I treatment: Functional appliance therapy with twin Block

Intra-oral problem list included an End-on molar relation on the right side and Class II molar relationship on the left with a single tooth cross bite w.r.t. 16. A severe deep bite with an exaggerated curve of Spee- a classic symptom of a skeletal class II was evident as well.
Twin black was delivered with an expansion screw due to the presence of a relative crossbite on mandibular advancement. Construction bite was made with the incisors in edge to edge position with single stage advancement planned. We were able to achieve a Class 1 molar relationship along with a significant bite opening and overjet reduction. 6 months post retention exhibited a more settled bite with the molar relationships maintained.
Now that the jaws are well aligned, its time to detail that occlusion to perfection with Phase II- Braces!

Appliance used: Twin block appliance with expansion screw.

Activation: One turn/week

Time of active wear: 6 months

Retention phase: 6 months

A stitch in time saves nine! (1/2)

A shy, sweet girl presented with a simple complaint- gap in the bunny teeth.

The abnormality, though was more than what met the eye.
On examination, she exhibited a Class II skeletal relationship with a retrognathic mandible (backwardly placed lower jaw) with a horizontal growth pattern with a Class II division 1 dental relationship. She had a deep mentolabial sulcus, deep nasolabial folds and a reduced lower facial height owing to the severe deep bite.
The beauty with treating growing kids is the possibility of harnessing active growth and redirecting it to correct jaw relationships- Eliminating the need for a possible jaw surgery as an adult. Isn’t that great? Or should I say almost magical? Timely intervention is key here- the bone responds faster in a younger age group.
Treatment plan consisted of a Removable Functional appliance- The Twin Block with the objective of advancing the mandible (lower jaw). The Visual Treatment Objective was positive- green lighting a Functional appliance as the most suitable treatment of choice.
Full credit to her- she was a very compliant patient. We were able to achieve excellent results in a short period of time- with a phenomenal increase in the lower facial height, significant bite opening, a flatter mentolabial sulcus and an advanced mandible.
6 months after the treatment, the mandible has truly stabilised in the advanced position, the growth re-directed successfully in our favour (that lower facial height) and our girl is also now a full fledged teenager all set to undergo her phase II treatment- Braces!
