Occurrence of Black Chromogenic Stains and its Association with Oral Hygiene of Patients
• Black tooth stain is a characteristic extrinsic discoloration commonly seen on the cervical enamel following the contour of the gingiva caused by chromogenic bacteria.
• It can be diagnosed as pigmented, dark lines parallel to the gingival margin or as incomplete coalescence of dark dots rarely extending beyond the cervical third of the crown.
• Though these stains are more prevalent among children, in the present scenario their occurrence has been noticed in adult population also.
• The presence of black stains were associated with low caries risk.
• It is a form of bacterial plaque and mainly consisted of black insoluble ferric compound, probably ferric sulphide.
• The presence or absence of these stains as
1) No line;
2) Incomplete coalescence of pigmented dots;
3) continuous line formed by pigmented spots.

g) Medications -such as corticosteroids, immunosuppressive, chemotherapeutic agents, long term use of low dose asprin

i) Respiratory Disorders – Asthma, COPD, Chest Infection
j) Extraction during menstruation period – Painful stressful condition

Commonly known as a “sickle probe” , Usually made of flexible metal that conducts vibrations from working end to the clinician's fingers . Working end is 1-2 mm of the side of the explorer called " The explorer tip " .

💠 Primary Designs of Dental Explorers 👇🏻
1. Cowhorn or Pigtail Explorer :
🔸️Uses : A) Calculus detection in normal sulci or shallow pockets extending no deeper than the cervical third of the root . B) The curved lower shank causes considerable stretching of the tissue away from the root surface .🔹️Examples : 3ML , 3CH , and 2A ____________________________________ 2. Straight Explorer :
Unlike the “curved” explorers, the straight explorer has no curvature to the metal endpoint. It slants straight at a downward angle .🔹️Examples : 6 , 6A , 6L , and 6XL explorers ____________________________________
3. Shepherd Hook Explorer //🔸️Uses : A) Supragingival examinations for dental caries ( Decay ) and irregular margins of restorations . B) NOT recommended for subgingival use because the point could injure the soft tissue at the base of the sulcus or pocket .🔹️Examples : 23 and 54 explorers ____________________________________ 4. Orban or #17/18 Explorer //🔸️Uses : Assessment of anterior root surfaces and the facial and lingual surfaces of posterior teeth . Difficult to adapt to the line angles and proximal surfaces of the posterior teeth . 🔹️ Examples : 17 , 20F , and TU17. _____________________________________
5. 11/12 type explorer :🔹️Long complex shank suitable for both anterior / posterior teeth with normal sulci or deep pockets. 🔹️Tip at 90 ° to the lower shank . _____________________________________ 6. Curved Explorer //🔸️Uses : A) Calculus detection in normal sulci or shallow pockets. B) Care must be taken not to injure the soft tissue base of the sulcus or pocket if the working end is used subgingivally . 🔹️Examples : 3 and 3A .

🟣 Periodontal curettes - focus on Gracey curettes :
Periodontal curettes are used in non-surgical periodontal therapy, to treat the patient’s early stages of periodontal diseases, such as periodontitis .

✅ These double-ended dental hand instruments . They are designed to remove bacterial plaque and calculus from supragingival and subgingival tooth surfaces. Meticulous scaling and root planing with curettes has been scientifically proven to be very effective to improve periodontal health .

🔹️The original Gracey series was developed in the 1930's by Dr. Clayton Gracey , a periodontist at the University of Michigan. 🔹️ They feature long shanks and unique blades , some with bends improving access to complex root surface morphology .

Gracey curettes are available in stainless steel or in carbon steel . Carbon steel requires more care as it rusts very easily and also wears away more quickly.

It can be identified by holding the terminal shank of the end in question , blade side lower , in a vertical position and viewing the blade portion head on.

There are 9 types of traditional Gracey curettes. Each of them is designed to access particular areas of the mouth and particular surfaces of the teeth .

• Gracey 1/2 : for anterior teeth, all surfaces

• Gracey 3/4 : for anterior teeth, all surfaces

• Gracey 5/6 : for anterior teeth and premolars, all surfaces

• Gracey 7/8 : for molars and premolars, buccal and lingual surfaces

• Gracey 9/10 : for molars and premolars, buccal and lingual surfaces

• Gracey 11/12 : for molars and premolars, mesial surfaces
