Patient complained to us about having mobile teeth, increased spacing and ability of teeth falling off by themselves without much pain. It was observed that he had severely mobile anteriors, inflamed gums and shift in his jaw midline. After an X-ray and a 3D CBCT scan, it was found, he had a large cyst for the upper right canine, which caused severe bone loss and migration of the adjacent teeth. He had prior root canals done to his upper incisors which were re-infected and had abscesses. He was explained all the above findings and also his treatment plan.
We extracted all his upper teeth. The cyst was removed (Root canal opening was done for the tooth in question a week prior) and bone graft was placed. The infected granulation tissue from the other 4 teeth sockets was also removed. His lower severely mobile teeth too were extracted but on a later day. A removable upper complete denture and lower partial denture were fabricated. These temporary dentures are for aesthetic and functional rehabilitation while the jaws heal, which takes around 6 months.
We will be later doing a fixed implant supported crown framework by “ All-on-4” either zygomatic supported or long posterior Nobel Biocare implants depending on how well his jaw recovers. For newly placed implants to integrate with the bone, it takes 6 to 8 months. Subsequent to which, the crown frame work will be done.
The after pictures are with the dentures, which completes the first stage of the treatment.
