This is what happens with typical "I don't care approach". This patient reported our clinic with a loose implant. She came with all the documents proving that she was treated by a Professor of Oral & Maxillofacial Surgery department of India's one of the most reputed Medical University. The size of implant was 4.2mm X 13mm, in a bone which was merely 2mm thick. She claimed that even an attempt of bone grafting was done, but all failed. The Professor denied her to revisit claiming it to be a Covid-19 period. Later on we took over the case and removed the implant, periapical debris and grafted the site with bone and PRF. It's a 2 days old case. We have given her a temporary prosthesis, to be removed with a fixed prosthesis on a later date.

A male patient around 36 yrs. visited our clinic with a fractured tooth no. 16. We did RCT to relieve him from pain, followed by metal post and core. The tooth was finally restored with a PFM crown.

A young girl about 15 years old lost her both maxillary central incisors. She wanted all ceramic crown only. The problem was we had to fix the metal posts first after RCT, (as fibre posts are not that much long lasting). The metal posts show themselves through All Ceramic Crown. Dr. Ritu took the challenge and using a specific technique she first made the metal post which don't show themselves through all ceramic. And then All ceramic crowns were cemented. See the result.

A young girl at the age of marriage represented herself like this. Teeth no. 11,21 were fractured. Orthodontically it was an open bite case but , the patient wanted immediate solution as the date of marriage was fixed. So we did RCT 11,21. Restored the tooth Structure of 11,21 with post and core and PFM FPD at 13,12,11,21,22,23.

A young girl 15 yrs old visited our clinic with Class II Div 1 malocclusion, and tooth no. 21 missing, wearing a temporary Removable Partial Denture which had moved her teeth apart and shifted her mid-line. The upper and lower canines were rotated. The patient was conscious about her aesthetics and did not want to remove her partial denture even during the treatment. So we decided to give her a Maryland bridge as an intermediate option till it was replaced with implant supported prosthesis. we fixed the Maryland bridge and started with Orthodontic treatment. When the orthodontic treatment was about to complete there were few more steps left before completion of case. We had to place the implant and the problem was hour glass shaped bone which was only 2.3 mm at its narrowest point & the implant to be placed was 3.2 mm X 13.0 mm. So we decided to expand the bone. With help of our bone expander kit we started to expand the bone slowly and gently. With the specific technique that we use we could achieve the desirable result. Later on we made a bone bed on the hour glass depression and contoured the bone and closed the bone graft site with help of PRF. The beauty of bone expansion surgery is that the complete implant body is well enclosed within the natural bone. The bone graft helped us to re-contour her bony depression and her facial fullness is back. Case is in finishing stage now. We will fix the final prosthesis and close all the remaining gaps.

Part 2

On 9th June 2020 we removed her braces, splinted and polished. We gave her a healing abutment. The case was very challenging as we had to fix a custom made 30* abutment. We fixed the abutment and then the prosthesis which is 3M LAVA Essential (Zirconia). The case has come up well and the patient is extremely satisfied.
The total time duration to complete the case was 1year and 4 months which could be three months short if there was no Lock down.
