Pit and fissure caries!
While they might appear only superficial on visual examination, are often much deeper than they seem! Usually go undetected until patient has pain or sensitivity.

Explorer is an excellent instrument to detect soft caries at the depth of these fissures.

Is this case there was delayed response on cold test indicating a need for root canal treatment!

Curved Canal wrt 48.

Patient came with acute pain, on examination iopa revealed deep dentinal caries with periapical widening.

First visit: Initial BMP and copious EDTA irrigation.
Second visit: Final BMP and Obturation.

Long standing periodontal bone loss and lingering pain response to cold test wrt 46 indicating irreversible pulpitis, advised RCT followed by periodontal therapy.

Single sitting rct performed and recalled for followup!
