A patient presented with pain and discomfort in the mandibular posterior region. Clinical examination, supported by radiographic evaluation, revealed a mesioangular impacted mandibular third molar with close approximation to the inferior alveolar canal. Based on clinical and radiographic findings, a diagnosis of mesioangular impaction was established.
Surgical disimpaction was performed under local anesthesia using an appropriate mucoperiosteal flap design. Buccal guttering was carried out to facilitate access, followed by controlled tooth sectioning and atraumatic elevation of the tooth. Care was taken to minimize trauma to the surrounding bone and to avoid injury to the adjacent neurovascular structures. The surgical site was irrigated and sutured.:
Postoperative healing was uneventful. The patient reported mild pain and swelling, which subsided within a few days with prescribed medications. No signs of paresthesia or other complications were observed during the follow-up period.
Conclusion:
Careful preoperative radiographic assessment and meticulous surgical technique are essential in the management of mesioangular impacted mandibular third molars, especially when in close proximity to the inferior alveolar canal. Atraumatic disimpaction with appropriate planning can result in favorable outcomes and minimal complications.

Infections from untreated upper molars and premolars can sometimes spread beyond the tooth roots into the nearby maxillary sinuses. This condition is known as odontogenic sinusitis, a sinus infection that originates from dental problems rather than typical respiratory causes.

Because the roots of upper back teeth sit very close to the sinus cavity, dental infections can easily extend into this space. Patients may experience facial pressure, persistent sinus congestion, bad taste, or discharge from the nose. Unlike common sinus infections, these cases often do not respond well to antibiotics alone.

Proper dental treatment such as a root canal or extraction is usually required to eliminate the source of infection and allow the sinus to recover.
