74 year old male patient presented barely 3 months after an open heart surgery with circumferential caries involving pulp in tooth 37.

Given the patient's medical condition, extraction was not an option. The cardiologist advised to use only plain lignocaine without adrenaline. He probably does not know that the effect lasts for a maximum of maybe 5 minutes! Had a really hard time with this vital lower molar.

Though difficult, we managed to isolate with rubber dam and teflon. A post was passively bonded to retain the core. A full coverage PFM crown was given to protect the tooth.

We frequently get fractures in teeth with long standing amalgam restorations. Some fractures extend way too deep into the bone, necessitating extraction.

But if the fractured margin can be isolated with a little bit of gingivectomy or even osstectomy, I prefer to try to save the tooth.

Sharing here my method of isolation and restoration of a tooth in which the mesiopalatal cusp decided to fracture off. X-rays in the last slide.

This patient was told by her previous dentist that "the root canal is done, you just need a crown now". She came to us for "just the crown" during her 2 week vacation😬😬

We had a hard time explaining to her that:
👉the coronal restoration was just a temporary one,
👉the most difficult part was yet to be done,
👉we don't know if the periapical radiolucency means a healing lesion or a developing one,
👉the mesial aspect was guaged way too much,
👉a post and bonded core would be the best option and that
👉the 3rd molar needed to be extracted.

The patient agreed for the permanent restoration with deep margin aquisition, but did not agree for the post and core. She has been given written instructions to get the 3rd molar extracted.

We managed to restore the tooth and delivered a crown with a tiny overhang which could not be corrected because of her schedule.

I really don't know why the previous dentist promised to restore this tooth instead of suggesting extraction.

Would you prefer extracting such teeth or restoring them?

Phew! This one was 10/10 on the difficulty scale😥😰😰

The tooth was vital, the victim of its neighbour, the wisdom tooth. Restored this one with bulkfill flowable plus bulkfill paste in snow plough technique.

How do you restore such vital second molars? I know some colleagues who do intentional endodontic treatment followed by crowns.

A case referred for retrieval of fractured instrument. Looked easy on the pre-op x-ray, but the tip was stuck in one of the apical ramifications and I did take some time to loosen it out. Treatment completed under dental operating microscope and referred back.

Very interesting case of child putting stapler pin inside tooth, which caused a perforation. We saved the tooth with a multidisciplinary approach. The tooth is stable and functional.

When a root canal disappears on a radiograph as you move apically, it is called a "FAST BREAK PHENOMENON". This happens when the main canal splits into 2,3 or more small canals that are not visible on an x-ray.

Why is this knowledge important for a general dentist? Well, if you see this in a pre-operative x-ray, you must look for a split in the canal at the mid-root level, otherwise your endo will fail.

In this case, it was a simple bifurcation. A dental operating microscope definitely helps to find the additional canal and also to conserve tooth structure as we manage the additional anatomy.
