_!! Case Report of the Month!!_
“ *Acute Necrotizing Gingivitis sec. to Pseudomonas Aeruginosa infection* ”
A female patient of age 42 years reported with severe pain in her oral cavity and was unable to open her mouth since > 1 week
Patient is a k/c/o Breast Carcinoma Metastasized to Liver for which radiotherapy was done and currently undergoing Chemotherapy
Patient also has a history of severe diarrhea after chemotherapy session 15 days back for which patient was admitted in ICU
On Examination
White Necrotic Slough noted Buccally around gingival margins and attached gingiva on Right Maxillary and Mandibular Premolars and also noted lingually on Mandibular anteriors with generalized gingival erythema
Bone denudation was observed lingually on right mandibular first molar and second premolar
Slough was scrapped with bleeding on probing
Severe Trismus was observed
Culture of the slough was done with the result positive for Pseudomonas Aeruginosa infection sensitive to all the antibiotics. Pathogen was also present in blood and stool cultures
Treatment:
Ciprofloxacin 750 mg twice daily was administered with 0.12 % Chlorhexidine Mouthwash empirically
Patient reported relief in pain after only 5 days of the treatment.
Complete remission of white necrotic slough was observed after 12 days of treatment with growth of healthy gingiva observed over the denuded bone in 45, 46 region.

Discussion:
In immunocompromised patients the differential diagnosis of oral necrotic lesions in the absence of necrotizing periodontal disease opportunistic bacterial infection is suggested ; the author emphasize the importance of culture examinations to select the appropriate therapy to the specific pathogen.
Similar procedures were carried out in this case as necrotic lesions in marginal and attached gingiva induced by P. aeruginosa were treated with systemic antibiotics based on the susceptibility profile.
