Impacted molars are commonly encountered in routine dental practice. Impacted teeth may remain asymptomatic or may be associated with various pathologies such as caries, pericoronitis, cysts, tumors, and also root resorption of the adjacent tooth.
Mesioangularly Impacted 47 & 48
Mesioangularly impacted 48 with roots-1/3rd completed, and occlusal table in line with the crestal bone level noted within the ridge.
The mesial cusp of 47, contacts the distal root of 46, 2mm inferior to its CEJ. An ill defined hypodensity suggestive of initiation of cervical resorption at the point of contact-correlate clinically. The distal occlusal table is partially above the crestal bone.
After careful examination, the decision was made to extract the lower third molar and then upright the second molar with elastic chains and attachments on occlusal surface anchored by bone screw placed in the ramus.
Under local anesthesia, bone screw (2x14mm) was inserted in the ramus of the mandible. Elastic chains, anchored by the bone screw, was activated by connecting the opposite end of chain to the attachments bonded on the second molar.
Four months later, the second molar was uprighted successfully.
