A Class II restoration should never be approached as
“just a filling.”

When marginal ridges are lost, the tooth becomes structurally compromised and more prone to cuspal flexure and fracture under occlusal load.

That’s why I prefer treating these cases as rehabilitation rather than simple restoration.

“Hot tooth” cases can test your patience, skills, and anesthesia limits. 🔥🦷

That lingering pain to hot stimuli, spontaneous throbbing, difficulty getting profound anesthesia… we’ve all been there during RCT.

One small addition that has genuinely helped me manage these difficult cases better:

Articaine 40mg/ml along with Dexamethasone 4mg/ml đź’‰

Why it helps?
• Better diffusion of anesthetic in inflamed tissues
• Reduced inflammatory response around the pulp

Some of the most commonly used pre-flaring tools in endodontics are:

Dentsply X-gates Glidden Drills — classic option for coronal enlargement and straight-line access. Effective, but needs controlled use to avoid strip perforation in danger zones.


Coltene Hyflex Edm Refill (25/.12) 15mm Or***ce Opener — flexible heat-treated NiTi or***ce opener that improves coronal flare while respecting canal anatomy. .uk

Pro-Taper Hand Files Pack of 6 — clinicians commonly use the SX file for coronal pre-flaring and reducing coronal interferences before shaping. us

OneFlare / Or***ce Openers — designed specifically to enlarge the coronal third and create smoother glide path entry.

File breakage isn’t sudden—it’s cumulative.
�Every canal, every curve, every sterilisation cycle adds up.

Hand files may take you through ~5 canal�Rotary files can go up to ~8 canals
-but only under controlled use (keep checking flutes)

Because here’s the reality:�Sterilisation may improve flexibility and cyclic fatigue resistance initially…�but it also reduces cutting efficiency and affects surface integrity over time.

So what really matters?

✔️ Track your usage�✔️ Respect canal anatomy�✔️ Watch for unwinding or dullness�✔️ Don’t ignore silent fatigue
