🦷 TMJ DISC — THE SMALL STRUCTURE BEHIND A BIG PROBLEM!

The Temporomandibular Joint (TMJ) disc is a specialized fibrocartilaginous structure positioned between the mandibular condyle and the temporal bone. It helps the joint move smoothly during opening, closing, chewing and speaking.

🔹 What is Anterior Disc Displacement?

When the TMJ disc moves forward from its normal position, it is called anterior disc displacement.

➡️ With Reduction:
The displaced disc returns to its normal position during mouth opening and then moves forward again during closing. This commonly produces a clicking or popping sound.

➡️ Without Reduction:
The disc remains displaced and does not return to its normal position during opening. Patients may experience restricted mouth opening, pain and difficulty chewing—sometimes described as a “closed lock.”

⚠️ Common complaints include:
• Clicking / popping sounds from the joint
• Pain in front of the ear or jaw
• Difficulty or limitation in mouth opening
• Jaw locking or catching
• Deviation of the jaw while opening
• Pain while chewing
• Headache or associated facial pain
• Changes in the way the bite feels

🦷 Dry Socket isn’t bad luck… it’s often preventable!

A successful extraction doesn’t end when the tooth comes out—it ends when the socket heals uneventfully. Here’s what every dental professional should remember:

✅ Why does dry socket occur?
Loss or premature breakdown of the blood clot exposes the bone, leading to severe postoperative pain and delayed healing.

🦷 Which cutting instrument should be used while sectioning a tooth?
Use a surgical carbide fissure bur (typically 701/702 or 703) in a high-speed handpiece with copious sterile saline irrigation to minimize heat generation and preserve surrounding bone.

🌿 Why is gingiva so important?
Healthy, atraumatic gingival handling helps achieve primary closure, clot stability, and reduced bacterial contamination—all essential factors in preventing dry socket.

🪡 What sutures should be placed after extraction?
Use 3-0 or 4-0 silk, polyglactin (Vicryl), or PTFE, depending on the case. Simple interrupted sutures are the most commonly preferred technique, while figure-of-eight sutures are useful when additional clot stabilization is required.
