Class V Restoration are very routine procedures. They can either be due to caries (1st pic) OR they can be due to Non carious reasons (2nd pic) like tooth brush abrasions ( i don't align to the concept of abrafaction).
Regardless of the cause, i like to pack teflon in the sulcus and adjacent interdental papilla region for isolation from GCF & Blood. saliva can be easily managed with suction since its on lingual side and the restoration is on buccal/labial side.
when i see caries on the buccal surface of maxillary molars, i take time to educate the patient the importance of brushing properly and the right technique. Many times, ascending ramus is too much forward and interferes with brush going far back. In such cases, the lower jaw has to be open partially & moved on the side of brushing while brushing. The same hold true if you as a dentist find it difficult to put your elevator or scalar or any other instrument while operating on the buccal of the maxillary posteriors.
