“It doesn’t hurt, so it must be fine.”

That sentence costs Indian patients more money than any other.

Here is the part nobody explains: the nerve sits at the centre of
your tooth. Decay starts on the outside and works inward. It has to
cross two entire layers before it reaches anything capable of
reporting pain — and both of those layers are silent.

Stage 1 — Enamel. A white or brown patch that hasn’t broken
through. Enamel has no nerve supply at all, so there is literally
nothing here that could hurt. Caught now, this can often be
remineralised. No drill.

Stage 2 — Dentine. Through the shell, and now moving much faster,
because dentine is softer. It spreads sideways underneath a surface
that still looks almost intact. A brief twinge with something cold
or sweet is the only hint, and it passes. Still just a filling.

Stage 3 — Pulp. Now it hurts. This is the throbbing, sleepless
stage. And if you wait it out, the nerve dies and the pain stops —
which almost everyone reads as recovery. Root canal and a crown.

Stage 4 — Root tip. The infection leaves the tooth and enters the
bone. An abscess forms below the root. Root canal, or you lose the
tooth.

Notice where the pain starts. Stage three.

So “waiting until it hurts” is not a conservative approach. It is a
decision to skip the two cheapest stages entirely and start
treatment at the expensive one.

A check-up finds stages 1 and 2. An X-ray sees between the teeth,
where most decay actually begins and where no mirror or torch will
ever reach.

No pain means it hasn’t reached the nerve yet. Not that there’s
nothing there.

Save this. Then check when your last examination actually was.

The toothache that kept you up for three nights is gone. You didn’t
do anything. It just… stopped.

That is not recovery. That is usually the nerve dying.

Pain is the nerve reporting a problem. Decay reaches the pulp, the
pressure of the infection cuts off its own blood supply, and the
tissue inside the tooth dies. The reporting stops. Nothing was
cured — the alarm was disconnected.

Here is what people never get told: the infection doesn’t leave. It
changes address. Bacteria inside the dead canal drain out through
the tip of the root and settle into the jawbone underneath. There
is no blood supply left inside that tooth, so no immune cell can
reach it. Your body cannot clear this one on its own.

The quiet phase can last months. Then it comes back — usually as
swelling, usually at the worst possible time.

Five things worth checking on a tooth that went quiet:

→ It has turned grey, brown or darker than its neighbours
→ It feels tender or sits “taller” when you bite
→ A small pimple on the gum above it, that drains and returns
→ A bad taste or smell you can’t brush away
→ No reaction to cold, when every other tooth reacts

And why the usual fixes don’t end it: painkillers mask the signal
and change nothing. Antibiotics reach the swelling around the tooth
but cannot get inside a canal with no blood flow — they buy time in
an emergency, they are never a treatment on their own. The canal
has to be cleaned out mechanically. That is what a root canal
actually is.

If a bad toothache quietly disappeared on you — this month or two
years ago — bring that tooth in. An X-ray takes two minutes and
settles it either way.

Pain that stops is not the same as a problem that is solved.

If you brush hard, you probably think of it as being thorough.

It isn’t. It’s the most common way people damage their own gums.

Plaque is soft — it’s a film, not cement. Removing it needs contact
and time, not force. Which is why a soft brush used properly beats
a hard brush used aggressively, every single time.

What pressure actually does is wear down two things that don’t grow
back.

First, the gum. It’s thin tissue at the neck of the tooth, and
years of sideways scrubbing pushes it down the root a fraction at a
time. You never notice it happening. You notice it when your teeth
start looking longer.

Second, the root surface underneath. Once the gum has moved back,
what’s exposed isn’t enamel — it’s a much softer layer, and hard
bristles carve a visible notch into it. That notch is usually where
the cold sensitivity is coming from.

The fix is unglamorous:

→ Soft bristles only. Medium and hard have no advantage.
→ Hold the brush like a pencil, not a fist.
→ Angle it 45° into the gum line. Small circles, no sawing.
→ Two full minutes. Time does the work, not pressure.
→ If your bristles splay within a month, you’re pressing far too
hard.

Using an electric brush? Let it sit on the tooth and move it
slowly. Most models have a pressure sensor — if it lights up or
changes sound, ease off.

Receded gum doesn’t come back on its own. But you can stop it going
further starting tonight.

“Masood se khoon aa raha hai? Garmi hai, thanda pi lo.”

There is no such thing as bleeding from body heat. There never was.

Bleeding is inflammation. Your ear doesn’t bleed when you clean it.
Your skin doesn’t bleed when you wash it. A healthy gum doesn’t
bleed when you brush it — and it doesn’t matter how firm the brush
is.

What bleeding actually points to is one of five things:

1. Plaque sitting at the gum line. The most common cause by far,
and fully reversible. Bleeding usually settles in 10–14 days
once it’s cleaned off properly.

2. Tartar that has hardened below the gum. No brush, floss or
mouthwash reaches this. It keeps the inflammation running
permanently, and it takes bone with it.

3. Blood sugar that isn’t under control. Gums are often the first
place undiagnosed or poorly managed diabetes shows itself. And
gum disease pushes blood sugar higher in return — the loop runs
both ways.

4. A nutrient gap. Vitamin C, B12 and iron all keep gum tissue
strong and able to repair itself. Bleeding gums plus mouth
ulcers plus fatigue is a combination worth a blood test.

5. A hormonal shift. Pregnancy, puberty and menopause make gums
react far more strongly to exactly the same amount of plaque.
Pregnancy gingivitis is common and safe to treat — delaying the
visit is the actual risk.

Every one of those is treatable. None of them are heat.

The part that costs people the most: when gums bleed, most of us
instinctively brush that spot less. That leaves more plaque exactly
where the inflammation started, and it digs in.

Clean it gently and thoroughly instead. If the bleeding hasn’t
settled in two weeks, that is your signal to get it looked at.

“Scaling se daant hil jaate hain.”

Every dentist in Delhi has heard this sentence. It is the reason
lakhs of people avoid a cleaning for years — and it gets the cause
and the effect exactly backwards.

Here is what actually happens.

Tartar does not sit politely on top of your teeth. It creeps down
along the root, under the gum, into the exact space where healthy
bone is supposed to be. While it sits there, your body’s response
to it slowly dissolves that bone. No pain. No warning. No swelling
you would notice. This part takes years.

By the time you come in for a cleaning, the tartar is doing two
jobs at once — causing the damage, and hiding it. It fills the gaps
and braces the tooth from every side, like cement. A tooth that has
lost serious bone can still feel rock solid.

Remove the tartar and the tooth finally tells you the truth about
how much support it has left.

The looseness was always there. Scaling was just the first day you
could feel it.

Same story with the gaps. Inflamed gums are swollen gums, and
swelling puffs up to fill the triangles between your teeth. Once
the inflammation settles, the gum returns to its real, firm
contour — and the space that was always there becomes visible.

Sensitivity after a cleaning is normal too. Root surfaces that
tartar had been covering are meeting cold and sweet for the first
time in years. It settles in a week or two.

So if your teeth felt loose after your last cleaning, skipping the
next one is the worst possible response. It means your gums and
bone levels need to be checked properly — ideally with an X-ray.

Save this for the family member who has been avoiding their
cleaning since 2019.
