Case Report - Immediate Implant Placement in Maxillary Anterior Region

Chief Complaint

The patient presented with multiple missing upper anterior teeth and was wearing a removable prosthesis. The patient desired a fixed implant-supported restoration with individual crowns on separate implants instead of a bridge.

Clinical and Radiographic Findings

Clinical examination revealed mobility of tooth 21. Radiographic evaluation showed significant bone loss around 21, indicating a hopeless prognosis.

Treatment Plan

* Immediate extraction of 21
* Immediate implant placement at 21
* Implant placement at 22, 23, and 24
* Flap elevation with simultaneous bone grafting and augmentation
* Removable provisional prosthesis during healing
* Connective tissue graft planned during healing cap placement to improve soft tissue esthetics
* Final prosthesis planned after 4 months

Surgical Procedure

A full-thickness flap was raised, and tooth 21 was atraumatically extracted. Implants were placed at sites 21, 22, 23, and 24. Simultaneous bone grafting and augmentation were performed to improve ridge contour and implant stability.

Healing and Prosthetic Phase

Healing was uneventful. After 14 days, impressions were made and a removable provisional prosthesis was delivered. Final restorations are planned as individual zirconia crowns layered with E-max ceramic for maximum translucency and esthetics.

The patient presented with an edentulous maxillary arch and a faulty fixed bridge with multiple failed root canal treated teeth in the mandibular arch. Clinical and radiographic evaluation revealed a poor prognosis for the remaining mandibular teeth, indicating the need for full-mouth implant rehabilitation.

Treatment Plan

* Maxillary Arch: Full-arch rehabilitation with an All-on-6 implant-supported prosthesis.
* Mandibular Arch: Removal of the existing faulty bridge followed by atraumatic extraction of the remaining teeth and immediate implant placement using the All-on-6 protocol.

Surgical Procedure

* Under local anesthesia, a full-thickness mucoperiosteal flap was raised in both arches to adequately expose the alveolar ridge.
* The faulty mandibular bridge was carefully removed.
* Atraumatic extraction of the remaining mandibular teeth was performed while preserving the surrounding alveolar bone.
* Alveoplasty was carried out to remove bony spicules and obtain a smooth, flat alveolar ridge contour for ideal implant positioning and prosthetic support.
* In the maxillary arch, indirect sinus lift procedures were performed in the posterior regions to achieve adequate vertical bone height for implant placement.
* Sequential osteotomy preparation was completed, followed by placement of implants according to the All-on-6 concept in both maxillary and mandibular arches.
* Adequate primary implant stability was achieved.
* Following implant placement in the mandibular arch, bone graft material was placed around the implants wherever indicated to enhance bone support and promote osseointegration.
* The surgical sites were thoroughly irrigated and primary closure was achieved using horizontal mattress sutures with Ethilon 3-0 suture material.

Post-Operative Phase

* Healing was uneventful.
* Sutures were removed after 14 days.
* Following suture removal, impressions were made for fabrication of the temporary denture/provisional prosthesis, which was delivered the next day.

Final Prosthetic Plan

* After an osseointegration period of approximately 5 months, definitive implant-supported PEEK prostheses are planned for both the maxillary and mandibular arches.

The patient presented with pain associated with old, bulky porcelain-fused-to-metal (PFM) crowns in the maxillary anterior region. Clinical and radiographic evaluation indicated the need for endodontic and prosthetic intervention.

The existing PFM crowns were removed to facilitate proper diagnosis and treatment. Root canal treatment (RCT) was initiated and completed with respect to teeth 11, 12, 21, and 22. Following biomechanical preparation, a calcium hydroxide intracanal medicament was placed for 14 days. This was followed by multiple sittings of copious irrigation to ensure adequate canal disinfection prior to obturation.

After successful completion of endodontic therapy and confirmation of symptom resolution, the teeth were prepared for definitive prosthetic rehabilitation. Zirconia-layered lithium disilicate (E-max) crowns were fabricated and cemented with respect to teeth 11, 12, 21, and 22, achieving improved esthetics, function, and patient comfort.
