“Sir, why are you so particular about scans and planning before surgery?”

A CT or CBCT scan isn’t something I recommend simply because it’s more advanced.

I recommend it when it can make surgery safer, more precise, and more predictable.

Three-dimensional imaging can help us understand the patient’s anatomy in greater detail—such as the position of important nerves, the maxillary sinus, bone dimensions and quality, and even certain fractures that may not be adequately appreciated on conventional imaging.

But here’s something many patients may not know:

CT or CBCT is NOT necessary for everyone.

During my postgraduate training, advanced imaging wasn’t routinely available. We relied heavily on clinical examination, anatomical knowledge and conventional X-rays to manage fractures, impacted teeth and implant cases.

Later, with panoramic imaging such as OPG, we were able to manage a wide range of cases effectively.

Today, we have much more advanced imaging—but more technology doesn’t mean every patient needs more scans.

The question should always be:

“Will this scan provide information that changes the treatment plan or improves patient safety?”

If conventional imaging is sufficient, that’s what we use.

If three-dimensional imaging can give us the additional information needed for a safer and more predictable procedure, then we use it.

Because good surgical planning isn’t about ordering more tests.

It’s about ordering the right investigation for the right patient at the right time.
