Well to do something right and with perfection generally takes time!!!!

Quite often I encounter this question in the practice. Patients think that composite fillings are one of the fastest and easiest procedures in dentistry and they shouldn't take much time.

However, there is science and skill involved in every procedure and step in dentistry that needs to be followed meticulously by the physician and these steps generally take time. Failure to follow these steps leads to increased chances of failure.
Composite fillings like these require the dentist to maintain normal contact with the adjacent tooth. If not done right may lead to problems for the patients in the future.

How do we know that the contact is maintained?
just run a string of floss between the tooth
either it will pass freely------------- Problem
it wouldn't pass at all --------------- Problem
it will make a "CLICK SOUND" ---------- That's how it's supposed to be

HOW?? The Answer to this question is Pain
Root Canal treatments are considered to be painful and horrifying. Well if done in the right way the pain can disappear or at least reduce down in a day and can make you gain the trust of the patient.
Well such patient presented to me with severe pain on the scale of 9/10 and she had been popping painkillers for past 5 days. First sitting of the Root Canal treatment was done and the patient was pain free.

A patient complained of decay associated with tooth no 17. The patient had got the RCT done locally as he couldn’t travel to our clinic in the lockdown however he didn’t move forward to get a crown done as he was unhappy with the experience of the previous treatment.

He waited a whole 2 years but in the bargain lost a lot of tooth structure to decay when he presented to the clinic. There was hardly any buccal wall left to the tooth structure and he was clear about saving the tooth. I had informed him regarding the status of his prior treatment and that he would require a Re-RCT. However, the tooth was asymptomatic for the past 2 years after the treatment and the patient opted out of RE-RCT so I moved forward with a Post &core and a full metal crown for the patient.

Understanding the patient’s need and providing the best possible treatment while informing the patient of every possible treatment plan he could opt for is the aim of the practice.

The patient presented with decay at a surprisingly uncommon surface of the tooth.
However, on excavation the decay was soft and the patient had one day to spare for the treatment as he had to return to the UK.

As a dentist, I have a very conservative approach toward any treatment. I believe in minimal destruction and doing the best I could for the patient.

Minimal use of rotary, hand excavation of the decay, and placement of a cavity liner for the protection of the pulp in such cases have proven to be a very effective methods in my practice.

"Even the smallest of a change makes a big difference"

The patient was conscious of the decay on his front tooth. He was a teenager for whom the decay seemed to be an embarrassment.

I restored the tooth with a composite filling with the very best of my efforts to make him happy.

Well, I guess I did. He had the biggest smile on his face after the treatment was completed.

Patient presented with continuous pain associated with tooth no 36. The tooth showed severe attrition and dentin exposure.
Pain on percussion was positive. On cold test there was lingering pain associated with that tooth. Patient got relief of pain after the first sitting of the Root canal treatment and said “I feel relaxed being in this clinic” Well that was something quite opposite to what we as dentist generally hear from the patients😂

Do all teeth require root canal treatment??

The answer is Not Always!!

The above patient presented with deep carious lesions on tooth no 33,34. No history of pain or sensitivity.
On x rays as a dentist it’s interpreted that the decay has reached the nerve and needs a root canal treatment. However on clinical examination and on periapical radiographs its suggested that these could be save by a procedure referred to as PULP CAPPING.

On clinical examination the following test were done
1.No pain on percussion
2.No probing depths
3.No history of any pain or sensitivity
4.Normal response to cold test

Steps carried out for the procedure-
1.Caries excavation
2. Placement of the biomaterial in this case MTA (Dentsply ProRoot MTA)
3. Sealed the biomaterial with an intermediate restorative material and observed the patient for any signs and symptoms pertaining to that tooth till i recalled the patient for next appointment.
4.On recall the patient presented with no history of pain and sensitivity. The tooth responded normal to cold test.
5.Final Placement of the composite restoration.

Scaling is a procedure done by your dentist to remove all the plaque, tartar(calculus), and stains above and below the gum line.Tartar build-up for a long period leads to a receding gum line and bone loss eventually leading to tooth mobility.

In the above patient, a lot of tartar build-up was seen on examination. In this case crowding of his teeth was the main cause as a brush fails to reach these areas. The patient complaint of bleeding gums on brushing and was unhappy about his aesthetics. However, his complaints were resolved after Scaling and Polishing was performed for him. Also, note the color of his healthy gum a few days after the procedure.

If you are looking for a toothpaste "for best protection", find one with a least '1000 parts per million fluoride'

Years of research proves that it prevents cavities!

In our country a lot of non fluoridated toothpaste are marketed on a bigger platform as people are ignorant regarding the benefits of fluoride as an ingredient.
