Bilateral maxillary and mandibular posterior edentulism treated with dental implants. Mandible was straightforward with axial implants placed freehand using implants from . Maxilla was a bit more complicated especially in the 2nd quadrant where the sinus had almost completely pneumatized leaving no scope for conventional implant placement without extensive sinus augmentation and grafting. An immediate placement was done in 25 with indirect sinus lift using minimal bone graft, a zygoma implant was placed at 26 and pterygoid implants were placed bilaterally, all with great torque above 60NCm. Case of full mouth implant rehabilitation using Pterygoids and Zygomatic Implants for a highly atrophic maxilla. Mandible was solid with good height and width.. Sharing with consent: Case of Road traffic accident, fall from a two wheeler with the face hitting an electric pole. Sustained deep facial lacerations which were sutured in the Emergency Room under local anaesthesia. CT scan revealed depressed comminuted Frontal bone (forehead) and undisplaced Nasal bone fracture along with minor brain contusions. Patient was taken for ORIF under General Anesthesia after Neurosurgeon's consent and a bicoronal flap was used to access the frontal bone, the depressed bone segments were gently elevated and fixed using 1.5mm miniplates. Nasal fracture was managed conservatively. Pictures are self explanatory.

Sharing with consent: Case of Road traffic accident causing extensive hemi-facial abrations and a fractured right zygomatico-maxillary complex (cheek bone). Fractures sites were exposed, reduced and fixed using Titanium miniplates under General Anesthesia.

Sharing a short case of abscess in the Submandibular space. This young man was referred to me by a fellow dentist who had started treating him for a long standing dental pain/decay which the patient had ignored. He had started getting a mild swelling about 15 days back for which he self-medicated with over-the-counter analgesics amd antibiotics and finally visited the dentist when it did not get relieved. Patient had no medical comorbidities, was afebrile but was in severe pain and unable to eat well. Incision and drainage was done immediately under local anaesthesia by a stab incision in the neck and approx 50ml or more of pus was evacuated. Wound was washed thoroughly and a corrugated rubber drain was secured to keep the cavity patent. Wound irrigation was done regularly and the drain was removed on the 3rd day as there was no evidence of pus drainage and the wound was closed.

Moral of the story: Do Not take dental pain/infections too casually and Do Not self medicate.

A young lady suffering from a swelling on the left side of her face came to us for help. Scan showed a large expansile "cystic" lesion with corticated borders involving maxillary sinus just short of touching the orbital floor, hard palate and also the left lateral wall of the nose along with involving multiple teeth causing rooth resorption and displacement of roots. It was diagnosed as Odontogenic Keratocyst (OKC) after biopsy. OKCs are highly aggressive lesions commonly occuring in the jaws with a high potential for recurrence. If left untreated, they can cause massive bone destruction and facial disfigurement. She was treated by complete enucleation of lesion + peripheral ostectomy + chemical cauterization using Carnoy's solution under General Anesthesia. Wound cavity was packed with BIPP soaked ribbon gauze which will be changed every 4-5 days to enable shrinkage of the defect and healing. Patient has been advised long term follow up.

Sharing a case of lacerated eyebrow in a 28 year old lady caused due to fall from a two wheeler. Wound was deep, through skin, muscle and periosteum and a small portion of the skull bone underneath was exposed. Wound was thoroughly debrided and closed in multiple layers under strict aseptic measures, using 5-0 Vicryl to close the inner layers and skin closure was done using very fine 6-0 Prolene suture in a subcutaneous fashion to minimise scarring and achieve a natural looking eyebrow. Antibiotics and analgesics were prescribed. Photographs show post injury, immediate post closure and 15 days post closure.

Case of fractured lower jaw in 2 places (parasymphysis and body) due to hit by a cricket ball in a young male patient. Fractures reduced under GA and fixation done using titanium miniplates. Patient was back to immediate jaw function post surgery.

Sharing a case of Internal Derangement of Temporomandibular joint treated by Arthrocentesis under Local Anesthesia. Conservative management which aims to provide relief from chronic joint pain, clicking during opening and closing movements and improves mobility. Sometimes multiple sessions are required in more severe cases.

Sharing a case of palatal fistula closure in a 12 year old boy. Closure was previously attempted by another surgeon about 6 months back using tongue flap which unfortunately failed. Attempted to close the same again using an anteriorly based tongue flap. IMF was done to restrict mouth opening till flap division.
