📋 PRE-TREATMENT PROTOCOL & STANDARDS OF CARE

1. Comprehensive Oral Examination & Medical History
Every new patient—or returning patient with a new complaint—undergoes a detailed extra-oral and intra-oral clinical examination. This includes a complete assessment of the temporomandibular joint (TMJ), muscles of mastication, oral mucosa, dentition, and periodontium.
We will also review your complete medical history, which includes:
· Any allergies (especially to penicillin, cephalosporins, or other drugs)
· Current medications (prescription, over-the-counter, or herbal supplements)
· Any gastric issues, acidity, or peptic ulcers
· Past surgical procedures (major or minor)
· Any ongoing systemic conditions such as diabetes, hypertension, cardiac diseases, respiratory disorders, or blood disorders
This comprehensive evaluation is mandatory for your safety and helps us customize your treatment plan to minimize risks.

2. 🚨 MANDATORY MEDICAL DISCLOSURE – Lifestyle & Systemic Conditions
This section is critical for your safety. Withholding any of the following information can put you at serious risk of life-threatening complications during or after dental treatment. Please disclose everything honestly—your information remains strictly confidential under medical ethics and law.

A. Chronic Smokers, To***co Chewers, & Chronic Alcoholics
· Smoking and To***co Use: These habits severely compromise blood supply to the gums and jawbone. This leads to delayed wound healing, a significantly increased risk of dry socket (osteitis) after tooth extractions, and higher failure rates for dental implants and gum treatments. To***co also masks early signs of oral cancer.
Our protocol: We will modify your treatment plan—this may include prescribing longer antibiotic courses, recommending platelet-rich fibrin (PRF) to enhance healing, and advising you to stop to***co use at least 24–48 hours before surgery and for 7 days afterward.
· Alcoholism: Chronic alcohol consumption increases bleeding risk during procedures, interferes with liver metabolism of anaesthetics and painkillers, and raises the risk of intra-operative bleeding. It also predisposes you to severe gum disease and oral cancer.
Our protocol: We strongly advise you to abstain from alcohol for at least 24 hours before and after any surgical procedure.

B. Heart-Related Diseases (CHD, Stents, Bypass Surgery, Valvular Issues, Hypertension)
· Patients with a history of Coronary Artery Disease, angioplasty/stents, open-heart surgery, or heart valve replacements are often on blood thinners (antiplatelets or anticoagulants). Stopping these medications abruptly can trigger a heart attack or stroke.
Our protocol: We will consult your cardiologist to safely manage your medication schedule during dental procedures. Never stop blood thinners on your own.
· For patients with valvular heart disease or prosthetic heart valves, we MUST prescribe antibiotic prophylaxis before any invasive procedure to prevent Infective Endocarditis—a potentially fatal infection of the heart lining.
· Uncontrolled hypertension (blood pressure > 160/100 mmHg) is a contraindication for elective dental surgery. We will delay treatment until your BP is stabilized under medical supervision.

C. Lung-Related Diseases (COPD, Asthma, Tuberculosis)
· Patients with Chronic Obstructive Pulmonary Disease (COPD) or severe asthma are at higher risk for respiratory distress during dental procedures.
Our protocol: We will minimize the use of rubber dams, avoid lengthy supine positioning, and keep emergency bronchodilators (inhalers/nebulizers) ready at all times.
· Dental infections can exacerbate COPD symptoms. Therefore, we prioritize early and aggressive management of any oral sepsis in such patients.

D. Bleeding Disorders & Blood Thinners
· Please inform us if you are on blood thinners such as Aspirin, Clopidogrel, Warfarin, Apixaban, Rivaroxaban, or any other anticoagulant/antiplatelet agent.
Our protocol: Do NOT stop these medications on your own. We will liaise with your physician to determine if a temporary dose adjustment is needed, or we will perform procedures using advanced local hemostatic measures to control bleeding.
· If you have any hereditary bleeding disorders like Hemophilia or von Willebrand’s disease, we will coordinate with your haematologist to ensure appropriate pre-treatment factor replacement therapy.

E. Any Previous Surgeries (Major or Minor) & Medical Reactions
· Disclose any prior surgical procedures you have undergone—this includes joint replacements, organ transplants, cancer resections, or even minor oral surgeries. Some conditions may require pre-medication (e.g., antibiotic prophylaxis for joint replacements).
· Report any adverse reactions you have experienced in the past—to local anaesthetics (e.g., fainting, palpitations, prolonged numbness), sedatives, analgesics, or any other drugs. This helps us choose safer alternatives for you.

F. Accidental Traumas & Congenital Diseases
· Any facial or jaw fractures, head injuries, or neck injuries from past accidents must be disclosed, as these may affect your jaw opening, TMJ function, or healing capacity.
· Congenital conditions—such as cleft lip/palate, cardiac septal defects, Down syndrome, or any other birth defects—may necessitate modifications in patient positioning, anaesthesia technique, or procedure timing. Please inform us of all such conditions.

G. Blood-Related & Infectious Diseases (HIV, Hepatitis B/C, etc.)
· We maintain strict universal precautions and sterilization protocols for ALL patients. However, knowing your status for HIV, Hepatitis B, Hepatitis C, or other blood-borne infections is vital—not for discrimination, but for your own safety and optimal care.
· This knowledge allows us to:
· Avoid prescribing hepatotoxic or nephrotoxic drugs that could harm your liver or kidneys.
· Monitor you more closely for delayed healing and increased risk of secondary infections.
· Use additional barrier techniques if needed, ensuring maximum protection for both you and our clinical staff.
· Your information remains strictly confidential and is protected under all applicable medical ethics and data protection laws.

3. Mandatory OPG (Orthopantomogram) & Diagnostic Imaging
In our practice, no invasive procedure—whether restorative, endodontic (RCT), surgical, or implant-related—is initiated without a recent, high-quality OPG X-ray (preferably taken within the last 6 months).
The OPG is not merely a formality; it is our diagnostic compass. It allows us to:
· Evaluate the quality and quantity of bone, root morphology, and proximity to vital structures (nerves, sinuses).
· Detect any previously failed or improper restorations, hidden caries, or periapical lesions (infections at the root tip).
· Assess the position and status of wisdom teeth and any associated pathology.
· Screen for TMJ bony changes or other incidental findings.
· Plan your treatment with precision and predictability.
In complex cases, we may recommend additional intraoral periapical (IOPA) X-rays or a Cone Beam CT (CBCT) for three-dimensional assessment.
Please note: X-rays and specialist Oral Medicine & Dental Radiology (OMDR) consultations are usually conducted at partnered diagnostic centres. Treatment planning is finalized only after we have reviewed the OMDR report and imaging.

Once we have a complete diagnosis, we will explain your oral health status in simple, clear language—highlighting the problems, proposing all viable treatment options, and explaining the advantages, disadvantages, and risks of each approach.
We will provide you with a detailed, itemized estimate of all treatment charges. There are no hidden costs.

5. Informed Consent – A Legal & Ethical Commitment
Patient autonomy and informed choice are paramount in our practice. No treatment—whether diagnostic or therapeutic—commences without your explicit consent. This consent can be given in the following forms:
· Verbal Consent: This is recorded and witnessed by a staff member after we have explained the procedure, risks, benefits, and alternatives to you. A note is made in your file.
· Written Consent: We will provide a formal consent form for your signature, especially for major procedures like dental surgery, dental implants, or root canal treatments.
· Implied/Financial Consent: Your initial payment towards the proposed treatment is considered a legal acknowledgment of consent, as it signifies your agreement to proceed with the treatment plan discussed. A copy of the signed estimate and payment receipt serves as documented consent.

Dosage: 1 – 0 – 1 (After meals, twice daily) x 5 days.
For pain relief and reduction of inflammation.
Take only after food to minimize gastric irritation.
☆☆2. Tab. Augmentin 625mg (Amoxicillin + Clavulanic Acid) - 10 tablets .
Dosage: 1 – 0 – 1 (After meals, twice daily) x 5 days.
This is an antibiotic. Please complete the full 5-day course even if symptoms improve, to prevent recurrence of infection.
☆☆3. BEPLEX – CZS (Multivitamin with Zinc) - 10 tablets.
Dosage: 0 – 1 – 0 (Once daily, preferably after lunch) x 10 days.
Supports immunity and aids in faster tissue healing and recovery.
☆☆4. Tab. Ketorol DT 10mg (Dispersible Ketorolac) - 10 tablets.
Dosage: 0 – 0 – 1 (At bedtime, only if needed) x 5 days.
Rescue medication. Take it only if you experience unbearable night pain. This is a strong analgesic; do not take it if you have a history of gastric ulcers.
☆☆5. MultiDent OR Mucopain Gel - 1 tube.
Apply topically as needed Gently massage a small amount onto the affected gum/tooth region if you experience localized swelling or pain.
☆☆6. Chlohex ADS Mouthwash (Chlorhexidine) - 1 bottle.
Rinse 5ml twice daily Swish for 3–5 minutes after brushing. Do not swallow. Dilute with water if irritation occurs. Best used 30 minutes after brushing.
☆☆7. Vantej Forte Toothpaste (with Fluoride) - 1 tube.
Use for regular brushing Provides sensitivity relief and strengthens enamel.
☆☆8. Sensodyne Ultra-soft Toothbrush - 1 piece.
Use with gentle, circular motions Extra-soft bristles prevent trauma to gums, especially during the healing phase.

· COMPLETE DISCLOSURE: Re-confirm that you have disclosed ALL lifestyle habits, systemic diseases (heart/lung/blood), surgeries, traumas, congenital conditions, and medications—especially blood thinners. Your safety depends on this.
