A 28 years old male patient came to our clinic having pain and swelling in the left upper first molar. According to him, he got treated for the same tooth at some other private clinic. Failed root canal treatment was done previously and then extraction was advised.
The patient came to our clinic for a second opinion. The radiograph reveals a broken file in the palatal canal which is extending beyond the apex. Clinical examination shows extensive pulpal cavity preparation which also leads to perforation of the tooth mesially. The mesiobuccal canal was unprepared. This extensive cavity preparation suggests vigorous attempts to retrieve the broken file by the prior dentist.
Considering the prognosis of the tooth and the benefit of the patient, extraction of the tooth was performed.
