Full Mouth Rehabilitation Case ✨🦷

A challenging case of generalized attrition with significant loss of anterior guidance, flattened occlusal curves, deep posterior occlusal facets, and severe wear in the lower anteriors.

Patient concerns included:

-Sensitivity to hot and cold
-Edge-to-edge bite
-Difficulty in chewing
-Functional and esthetic compromise

Treatment plan:

✔️ Diagnostic models, clinical photographs & digital pre-op scans
✔️ RCT done for 47
✔️ extraction of 18 & 28
✔️ Patient education using diagnostic wax-up
✔️ APT / mock-up test drive for esthetic and functional evaluation
✔️ Vertical Dimension of Occlusion (VDO) increased by 3–4 mm using Lucia Jig
✔️ Tooth preparation completed in two stages
✔️ PMMA trial restorations given for 2 weeks using a fully digital workflow
✔️ No post-VDO symptoms or discomfort
✔️ Final rehabilitation completed with monolithic and layered zirconia crowns

Clinical Observations:

Multiple missing posterior teeth leading to difficulty in chewing.

Generalized attrition with loss of anterior guidance, occlusal curves, and edge-to-edge occlusion.

Attrited maxillary anterior teeth with periapical lesions in relation to 22 and 23; trauma-from-occlusion–induced mobility in the mandibular anteriors with associated bone loss.

Absence of bone in the right posterior maxilla.

Limited available bone in the left posterior maxilla.

Treatment Plan:

Diagnostic models, clinical photographs, and digital pre-operative scan records.

Extraction of 24, 25, and 26 followed by immediate implant placement in 24, 25, and 26 with indirect sinus lift.

Implant placement in relation to 45, 46, and 47.

Extraction of 15 and 16. Root canal with 22 23 17 

After a healing phase of 3 months, digital scans were taken. A full-arch pattern trial was performed to provide a functional and esthetic “test-drive” for the patient.

PMMA trial completed using a fully digital workflow.

Final restoration replicated using CAD-CAM milled ceramic (PFM / suitable final material).

Observations Clinically:
1. Multiple missing posterirors, difficulty in chewing, 

2. Genralized attrition and loss of anterior guidance, occlusal curves, edge to edge  occlusion. 

3. Upper attrided teeth with PA lesion 2223, TFO induced mobile lower anteriors with bone loss 

4. No bone on right side posterior maxilla. 

5. limited available bone in left sider post maxilla

 We planned:

Diagnostic models-photographs , digital preop scan record,

Extraction of 24 25 26 and immidiate Implant with 24 25 26 with indirect sinus lift 

Implant with 45 46 47 
     extraction of 15 16, 

After healing phase of 3 months,
Digital scan taken, Full arch pattern trial taken for giving test-drive to the patient and for esthetic-functional evaluation

5. PMMA trial with fully digital workflow.
6. Replication of same in final cad cam mls pfm Ceramic.

🔹 A 16-year-old patient presented with pain and a large periapical lesion in a previously root canal–treated tooth.
🔹 Re-root canal treatment performed with precise 3D cleaning and Metapex dressing.
🔹 After 21 days, obturation completed with excellent bone regeneration seen radiographically.
🔹 The tooth was then restored with an E.max onlay crown for long-term strength, function, and esthetics.

Case (OSMF STAGE 3)with All on six implants in Mandible, with immediate loading PMMA prosthesis and final Screw Retained on Ti BASE MLS with

Patient complain of

• Missing Teeth, Broken teeth in Lower Jaw
• Partially Edentulous both jaws, Few fractured anterior teeth and infected molar present with Bone Loss in lower jaw.
OSMF grade III with two finger mouth opening.

What we did:

• Six Implants in Lower arch placed under local anesthesia
Osstem Taper kit used.
All on six Implant placed Free hand without any guide with good primary stability >35 NCM, bone grafting and multiunit abutments anchored with anterior four implants, closed tray impression taken. Posterior implants on both side submerged.
Jig trial and bite taken at second phase (3rd Day)
Teeth setting trial done at third phase (at 5th day)
Immediate loading With Final milled pmma on multi unit abutments (at 7th day). All screws were tightened at 20 NCM torque. Only Anterior four implants immediate Loading done.

Prosthetic Phase (At 2 months )

Trios 3shape used for digital scan.
CAD CAM Milled MLS Framework
Lower jaw Screw retained MLS PFM on titanium base (porcelain fused to metal ) prosthesis given.
All implant screws are tightened at 20 ncm.

An Amazing FMR case with All on six implants in both the jaws, with immediate loading prosthesis and final Screw Retained comes to an End treated with Dental Implants
Patient complain of

• Missing Teeth Mobile teeth
• Partially Edentulous both jaws, only few anterior teeth present.
CBCT ANALYSIS cbct showing very Poor quality D4 bone in maxilla, with sinus dropped in molar area in both the side.

What we did:

• Six Implants in Upper and six implants in Lower arch placed under local anesthesia
Osstem Taper kit, Osstem All on 4 kit and sinuslift kit used in maxilla for direct sinus lift.
First phase upper jaw surgery,
All on six Implant placed with good primary stability >35 NCM, bone grafting and multiunit abutments anchored with closed tray impression taken.
Second phase lower jaw surgery,
All on six Implant placed with good primary stability >35 NCM, straight multi unit abutments placed with closed try impression and bite taken. (3rd day )
Teeth setting trial done at third phase (at 5th day)
Immediate loading With Final milled pmma on multi unit abutments (at 7th day).

Prosthetic Phase (At 4 months )

Trios 3shape used for digital scan.
CAD CAM Milled MLS Framework
