While evaluating a CBCT scan, an incidental finding was noted in tooth #36: extruded root canal sealer located superior to the inferior alveolar canal, showing a “wandering” pattern above the canal, without evidence of pe*******on. The patient reports no altered sensation.
From a risk assessment perspective, injury to the inferior alveolar nerve following endodontic treatment is possible.
The radiographic appearance suggests that the sealer has diffused through cancellous bone rather than encroaching on the neurovascular bundle. Therefore, no immediate intervention is required.

מוצגים כאן שני מקרים נפרדים של הסתיידות של תעלת השורש, Pulp Canal Obliteration PC, שהופנו אליי לצורך המשך טיפול לאחר כישלון באיתור תעלת השורש. בכל אחד מהמקרים קיימת אבחנת פריאפקס שונה.
בשני המקרים הייתה סטייה של חלל הגישה לכיוון בוקאלי כפי שניתן לזהות בהדמיית CBCT. בשני המקרים, התעלה נמצאת אפיקלית לגבול העצם הקריסטלית. טיפול במקרה מחייב ידע, ניסיון , יכולת לקרוא מפת דנטין, ושימוש בהגדלת מיקרוסקופ.
חשוב להדגיש כי לא כל מקרה של אובליטרציה של תעלת השורש מחייב ביצוע טיפול שורש, אלא אם כן מופיעים סימנים פרי-אפיקליים.
בשני המקרים, הטיפול הנבחר היה טיפול שורש אורתוגרדי. אולם, ישנם גם מקרים שבהם אנחנו בוחרים בגישה רטרוגרדית על פני אורתוגרדית, לפי הערכה של המקרה.

Endodontic microsurgery on upper left first molar. MB root with an apical curvature and a missed MB2 canal.
DB and P roots are fused at the apical third.
Resection of the roots was done beyond the "conventional" 3 mm in order to receive a better access and to avoid sinus pe*******on

Done for today !!!!

Tooth #37 diagnosed with Pulp Necrosis and Chronic Apical Abcess ( drainage through a 12 mm pocket )
Treatment included a long term intracanal calcium hydroxide medicament.
10 month follow up shows resolution of the lesion. Probing 4 mm at all sites

Endodontic Micro re-surgery
patient was referred for treatment of tooth #11, which had previously undergone apical surgery using the conventional technique. The procedure included elevation of a full-thickness flap, osteotomy, "shaving" of the root apex, and repair of the bevel (a standard 3-mm apical resection was not performed as expected). Retrograde cavity preparation was carried out up to the metal post and filled with BC putty
