One of the most complicated cases of 2025!
LL5 with deep split in apical third. Made trickier by the mesial inclination. Apical lesion developed between consultation and treatment. Thankfully no cracks discovered.
3 canals apically, 4 paths of exit. 20.04 shaping. Activation of irrigants and warm vertical obturation. Caries removed and glass fibre post placed.
Review after 3 months shows healing progressing well. Back to referring dentist for full coverage

Apical surgery LL6
Symptomatic AP from mesial root of LL6 + buccal swelling. Extruded file fragments apically.
Full thickness flap reflected. Root resection + retro preparation ultrasonically
Calcium silicate putty root end restoration
All nearly healed 9 months later
Pre, Post operative and 9 month review radiographs

UL7 crowned approximately 3 years ago. “Never felt right”.
Sinus tract present distobuccally - explained by the MB anatomy!
Treatment completed through restoration over 2 visits.
Many handfiles were sacrificed during this treatment!
4 canals - 2 paths of exit - MB1/MB2 communicated as well as DB/Pal
Endoperfection Neo 04 preparation - 25mm+ lengths
Hypochlorite + EDTA irrigation - Ultrasonic activation
Single cone cold condensation with bioceramic sealer obturation
Access restored with SDR and Composite

2 visit treatment - Calcium hydroxide dressing between visits
Threaded posts removed with ultrasonics, separated files bypassed in MB canal.
Separated file in DL canal removed ultrasonics.
4 Canals, 4 exits - Waveone preparation.
Hypochlorite + EDTA irrigation - Ultrasonic activation
Warm vertical obturation - Bioceramic sealer
Glass fibre post placed in DB canal + Rebilda core
Good initial healing - Back to referring dentist for full coverage
