Quite a rare one! This upper second molar was referred for root canal treatment due to deep carious lesion that caused pulp necrosis. Four roots with four independent canals were found (incidence 1.2%).

Shaped with martensitic files, copiously irrigated with NaOCl and EDTA activated before sealing 3 canals with CWC and the second distal with the sq**rt technique as the weird angle of the canal or***ce would have forced me to remove too much dentine.

Immediate restoration with fiber-reinforced composite. Pre-endo by
