Distal split identified on pre-op CBCT, although one of the three exits wasn't found or instrumented, and is only a chance capture. Resin and embedded glassfibre weave.

Splits. We did the selective retreatment of the first molar last month. We hoped the second one would do with a filling, but it started to act up before it got sorted.
I had spotted the MB split on the pre-op CBCT, but it's the continuous bleeding from the DB (see photos on the last slide) after debridement of the main canal that prompted me to scout for uncharted anatomy there.

Back to el grindo with this half-necrotic, half-IP LL7.
Precurved 10K in the M4 for debridement, sq**rt obturation of the distal root, cones in the mesial one.
The tricky bit was accessing slightly off to the distal so as not to damage too much whatever actual tooth was left behind.
I had planned to remove the excess material between the 2 teeth, but my predecessor will look after it in the end.

Thank you to all who responded to my stories. I agree with most of you regarding management, it's an easy, rapid and not-so-invasive surgery needed to assess and treat if suitable, which it was. However I am still puzzled as to why would part of the gutta percha separate and swim out of the root some time between 9 and 13 years post-op as the periapical bone breaks down.
Asymptomatic and functional at the 12-month recall.

The J-shape fallacy. Rarely if ever a sign of root fracture. I don't know how is this idea still sticking so hard in 2026, and can't imagine how many teeth are still being extracted because of it.
Under-obturated anatomy, no visible crack on the tooth, this is a no-brainer recommendation for retreatment in my book.
The path of least resistance for drainage of infection very often presents as a J-shape radiolucent area, in mandibular teeth at least.
Asymptomatic and functional 12 months out.

Rahim Karmali 's cases make me hope some of these teeth will exceed our expectations in terms of longevity.

There are a lot of questions, interest, and misunderstanding about conservative endo and MIE : this will be a 8-hour long, comprehensive lecture on the subject, where we will address all philosophical, theoretical and practical aspects.
