No time to manage the mesial margin, will do when we retreat the first molar, or when the RD preps it, whatever comes first.

SIP through cervical caries. I treated the first premolar about 8 years ago - doing well despite the vanishing Bioroot. Preop CBCT shows the caries into the pulp, and a buccal-oriented lateral canal.
The canal was accessed occlusally and debrided quickly before tackling the cervical caries and restoring this part. Then the RCT was completed and the tooth fully restored.

The main challenge is not to be iatrogenic. I couldn't get the main exit patent, so the trick is not to make things worse and feel when to stop trying.
Let's not pretend irrigation has anything to do with the immediate or current outcome.

The brickest brick. 80yof, SAP. It was horrible. I was digging and digging with the ultrasonics, breaking up calcifications only to find more. Got a touch of hope when I found the DB canal. A miracle happened and I eventually found the MB canal too. But I went too far down what should have been the palatal canal with no luck, so much that I decided I wouldn't dig any further so as not to destroy the tooth and started to obturate the buccal canals. Once done I went back a last time toward the palatal and got in instantly.. I honestly thought I had destroyed the tooth in the process, and was pleasantly surprised to see on the post-op xrays that I still had a good bit of margin after all...

Accessing through Turkey teeth. This has got to be my weirdest access to date. I drew a bunch of lines on the pre-op radiograph and CBCT to guide me to the canal after I figured the start of my access had to be dramatically mesial. I felt very uncomfortable drilling at such an angle into the crown, but caught the canal spot on.
