Control infection and prevent coronal leakage 🦠
Pt came with massive destruction of the coronal structure and hyperplastic pulp, after removal of inflammatory tissue, multiple perforation sites were revealed,
PA showed large radiolucency at the furcal and apical area,
Prognosis was questionable and accepted,
Single visit was done with MTA closure,
6 mos FU showed asymptomatic and rapid repair of furcal/apical bone, ready for indirect restoration 🫣

MM join with ML, Distal canal split and full of isthmus complexity,
Single session with high magnification,short reciprocal file,
There is no CBCT but 2-D shows the proximity of the IAN, strictly WL control,avoid excessive pressure obturation,

Multiple sites of pulp floor and canal wall perforation,
Huge MTA, circumferential depp margin elevation, ceramage Endocrown,

The huge lesion is healing..
Pain on biting at previously treated 36, PA finding was coronal leakage, short obturation (mesial), sealer extrusion (distal), furcation lesion and huge apical lesion with unidentified foreign object.
After the discussion, patient accepted the prognosis and treatment plan.
Two visits conventional rtx, redefining all the working length, finding another distal canal (+ brushing all canal wall), + 2mm or***ce barrier.
9 months follow-up: asymptomatic, tiny chipping composite, furcal/periapical bone remodeling, extruded sealer/foreign object disappeared (cbct to confirmed).
Pt suggested for indirect restoration and longer follow-up.
Conclusion: Bacteria were the main reason for this failed rct 🦠

The scan revealed a periapical radiolucency close to the inferior alveolar nerve (IAN) and a 26 mm long-curve radix entomolaris
Two visits with triple antibiotic paste; pain and facial swelling gone at 2nd visit
Void formation caused by insufficient compaction (rapid follow-up)
