Control infection and prevent coronal leakage 🦠
Pt came with massive destruction of the coronal structure and hyperplastic pulp, after removal of inflammatory tissue, multiple perforation sites were revealed,
PA showed large radiolucency at the furcal and apical area,
Prognosis was questionable and accepted,
Single visit was done with MTA closure,
6 mos FU showed asymptomatic and rapid repair of furcal/apical bone, ready for indirect restoration 🫣

MM join with ML, Distal canal split and full of isthmus complexity,
Single session with high magnification,short reciprocal file,
There is no CBCT but 2-D shows the proximity of the IAN, strictly WL control,avoid excessive pressure obturation,

Huge MTA, circumferential depp margin elevation, ceramage Endocrown,
3 months follow-up shows asymptomatic, rapid bone replacement and needs more years of follow-up 🫣
