A dear colleague came to see us with the long-standing problem of not being able to chew with her lower left teeth, but they are sensitive even when brushing.
Her fillings were changed several times but unfortunately the problem was not solved. At first glance, there was no major problem with the fillings, the dentin was a little free in the first molar, but the sensitivity was still present in the second molar too.
Our assumption was that the problem was caused by unsealed dentin tubules, so if we treat the teeth properly with adhesive the sensitivity would disappear.
We decided to replace all the fillings at this site.
The fillings were removed in one piece, so it was confirmed that poor adhesion to the dentin could have caused the sensitivity.
At the molars, a series of increasingly wide preparations left thin cusps where I suggested a careful reduction of the cusps in the hope of closing the preparation circle here.
Our patient is also very dedicated to her work and a big fan of biomimetic dentistry, so we decided to go for a completely stress-free treatment and even planned to inlay the tiny premolar.
Within days of the first session where the biobases were done, she reported back that all was well with her teeth, the sealed dentin tubules were fine.
One more thing to point out about the case. We have sent the lab a scan and impression, leaving it up to them to work on what they can do better.
They chose the impression and did such an accurate job that we didn't have to adjust anything on them.
