🦷 LIMITED NHS AVAILABILITY 🦷

We have limited space for 50 new NHS patients to join our practice on a first come first serve basis for appointments in July!

How to join the practice:

▪️Visit the practice between the hours of 10am - 1pm or 2pm - 4pm ( unfortunately, we will not be able to accept new patients outside of these hours )

▪️Fill out a medical history form ( please bring any relevant information that will be needed, e.g. your GP’s name and address, a list of current medications etc )

📋 Medical history forms must be filled out in practice - unfortunately these cannot be taken out of practice. Once a medical history form is filled out, hand to one of our reception team and Meg our Reception Lead will be in touch to book an appointment!

📆 Appointments will only be available on Wednesdays, at this moment in time

❌📞 Please bear in mind we will NOT be accepting any phone calls or emails in regards to new patients — you must attend the practice
