Here’s how I designed a space maintainer to allow the lower right premolar to erupt.
Both primary molars were submerging so I recommended they were extracted. But I was worried the space might be lost before the premolar erupted so fitted a space maintainer. The aim was to allow the premolar to erupt, the. Reassess for further treatment and also make a decision how to deal with the missing premolar on the left (leaving the primary molar wasn’t an option due to the submergence)

Bands on the first molars are joined by a lingual arch. An extension from the right first molar to the first premolar gives additional support, plus means that either part can be removed in future giving options as the premolar erupts. Two months after loss of LRE the premolar is starting to erupt

These are sintered bands made from a 3D intraoral scan so they fit the teeth perfectly, and there’s no need for separators before band fitting

Recently completed this fabulous treatment in Wetherby for H. His teeth were crowded and prominent so he wore twinblocks (removable functional braces that hold the jaw in a forwards position) first, followed by fixed braces. The great thing about this treatment is the improvement to the profile and there was no need to extract any teeth.

A simple treatment for a big change in an 8 year old.

This young boy had a crossbite of the front teeth and the back teeth on the right side. This is an ideal case for treating early, when there is still a mix of adult and baby teeth, especially when the jaw has to change position (in this case move forwards) in order to bite fully. This will help future develop, reduce the risk of teeth wear on the front teeth, improve appearance and make future treatment simpler.

I fitted an upper removable appliance-think traditional acrylic plate with metal clasps and springs-who ch incorporated a screw to push out the back teeth on the right. Springs behind the front teeth push them forwards. And bite blocks at the side separate the bite to allow the teeth to move.

This was worn full time for 6 months then at night for another 3 months. The anterior crossbite will self-retain, but he’s continuing with the brace at night for now to hold the expansion.

At this stage I’ve decided to accept there is still one incisor in crossbite as the rest of the bite is normal but there’s not quite enough space for it. Once all the rest of the adult teeth come through (2-3 years) then I’ll reassess for fixed braces to complete treatment.
