ALIGN, BRIGHTEN, CONTOUR

A simple case of clear aligners, whitening and some composite replacements to finish this case.

The composites were actually a lot bigger than they looked due to the fact that there was some staining underneath the previous restorations

COMPOSITE VENEERS

One of my favourite cases of all time. The change in this patient is unbelievable.

I put the profile shots on my story a while ago and I don’t think I’ve had as many comments on anything since then.

Patient had severe tooth wear and had lost a lot of teeth. When he presented he was looking for a change that wasn’t too false, but also not going to cost an arm and a leg.

In I step with some composite bonding and a denture made by the local clinical denture technician, Mike Rosa, to replace the missing teeth and here he is 6 months later showing things off.

I’ve included the occlusal pictures showing the palatal build ups just for any dentists who might be interested. I placed these freehand using a unica matrix held in with some liquid dam, and then used a leaf gauge to get the occlusion right

COMPOSITE BONDING

This case had finished some aligners a little earlier than I’d have liked but they really didn’t want to have any more, which is what happens with clear aligners, you always need more!

I placed 2 composite veneers on the centrals to restore the worn edges and to correct some minor crowding left after finishing the orthodontic treatment a little early

COMPOSITE VENEERS

Restoring function and aesthetics with composite is a great way to make people feel better about their smile, while making sure that things will last.

Without restoring function, things tend to break off, especially with composite. It keeps patients happy as it keeps their smiles looking great for longer

FIXED ORTHO

At the consultation I had warned the patient I might not be able to close these spaces completely and might need to bring them close enough to add composite without the teeth appearing too wide.

After about 6 months we finally got there and I didn’t have to place a single composite

COMPOSITE BONDING

Another subtle change here but this was more than it seemed.

The aesthetics have improved but there was a reason this patient had been chipping the same composite off for years and we needed to rethink things before trying the same as every other dentist.

I started with getting the appearance right so that is what you see here. I then had to add some composite (you won’t be able to see it) to the back of some teeth to re-jig the bite in the hope that if I changed the way this patient’s teeth fit together, the nice new restorations wouldn’t start to fail after a few weeks

COMPOSITE VENEERS

I placed the veneers on the first photo about 4 years ago when I was just starting to place them.

I’ve since changed a lot about my technique and honed it so that I can reproduce similar results every time. I feel like things have improved a lot since then and I usually say to patients that I expect 5-10 years with some repairs over that time but to be honest, I haven’t been doing them long enough to know for certain. I know the ones placed today will last much longer as I know how to avoid failures more than I did.

The first set had a few repairs over the 4 years but nothing major and the main reason for the change was the shade, the patient wanted them brighter, so enjoy!

COMPOSITE VENEERS

Kept this quite natural and I was made up when the patient asked for a change in shape and not shade.

I do love dramatic changes but I think I prefer the subtle ones over everything else

COMPOSITE VENEERS

Patient asked for a really bright smile with a correction of the mild crowding anteriorly but wasn’t keen on any ortho.

Sometimes this can be quite difficult to do, and in this case I struggled to get the contact straight and I faffed about adding and taking some away until I was happy. The issue with this was the position of the centrals, when they’re not straight it can hard to get the contact to appear straight. If I just placed composite onto the front of the teeth I’d have ended up with a wonky set of central incisors.

Sometimes you’ve just got to spend a bit longer making sure things are right before you move on to the next tooth 🤷‍♂️

COMPOSITE VENEERS

This was actually a multidisciplinary treatment because I provided a fixed brace for 6 months before placing composite veneers.

The pre-op photo shows how uneven the edges of the teeth and the gum lines were. I did offer the patient full rehab with composite but decided it best to place composite in an even thickness on each tooth so went with the fixed brace beforehand to make sure everything was well aligned.

This helped massively with the gum lines as I knew the edges didn’t need to be perfect because we had planned to do composite from the start
