SHOUT OUT TO THIS CLAMP

I've just recently started using a different range of clamps , one of which was the W56 universal molar clamp.

What's different? The bow extends further distally, which makes it easier to get a matrix band on while the rubber dam is in position. I almost exclusively use sectional matrices unless there are no neighbouring teeth which require tight contacts and marginal ridges. Quite often the cases where there is no neighbouring dentition can also be difficult to isolate.

Really looking forward to using this clamp more often, particularly for those more difficult situations.

Marginal ridges are an art form and a science I'm doing my very best to perfect but feel there is always room for improvement. While a variety of sectional rings, matrix bands and wedges are crucial to adapting each contact to each clinical situation, the instruments used to help form it all can be just as important.

For a while now I've been really happy with a microbrush and probe but I've been giving this composite instrument from a go! kindly lent me the instrument to get to grips with and so far it's been great! The Optrasculpt pads seem to be everywhere with cosmetic bonding cases and seem to be pretty tried and tested. The ball ended and pointed tips seem to work great for posteriors and have right profile to help shape those marginal ridges.

One thing I would love to see would be a composite brush attachment to help finish off everything!

Awkward cusps! The difference between a lovely straightforward class 2 restoration and the one which is a perfect to get right is the prominence of the cusps.

Without enough of a buccal/lingual wall the sectional ring just won't seat without affecting the matrix. I've tried buccal mall stamps (prior to the removal of old restorations), putty matrices and all sorts but none seem to give a seamless flow which allows for a perfect marginal ridge and effortless emergence and embrasures.

I'm quite keen to try the extra wide matrix ring from garrison, I hope this will help simplify the process.

It can be a pain when you lose a filling. Most of the time we forget about them until they're gone - then food starts getting stuck, teeth can become sensitive and we're at risk of allowing decay to develop in the tooth . This could lead to a whole host of other issues like toothache, pain, infection - all which might require further treatment such as a root canal treatment or extraction to solve the problem.

This patient came to The Tooth Spa after they lost part of their filling, they were struggling with the sensitivity and had concerns that things could get worse. Although there was some decay present we were able to replace the old metal filling with a white tooth coloured filling which helped to settle the discomfort.

White fillings are bonded to the tooth which can help to reduce the chance of the tooth breaking further in future. They do require more time, equipment and technique but are worth it for the benefits to the tooth long term!
