Post removal - This patient was initially looking into implant treatment but was put off by possibly needing bone grafting. She opted to keep her teeth, her UR2 has been treated with a post but no GP in the canal space beyond this. Post removal can sometimes be a tricky business. This post was a lab made post with a gold core. Once removed the canal space was readily instrumented and a fibre post and core placed with a temporary crown. She now has new Emax crowns and is happy with her choice. Her periodontal condition is also now stable.

Poor prognosis? Probably. This was a 36 year old female who attended as a referral from a local periodontist who was treating her for aggressive perio. In cases like this it is important to try and save as many teeth as possible. Ordinarily I would use MTA in this distal root owing to the likelihood of an open apex which reduces prognosis further; however, in this case after apical gauging it was noted that the apex was relatively small (ISO 25) and so it was treated with WoG medium (35/07). Coupled with a nice coronal seal we have given this tooth the best chance of survival in poor circumstances.

Converging canals. This was a case referred to me prior to the patient having roots removed and implant therapy undertaken. The patient was keen to save the UL4. As you can see on the preop there was a very large composite restoration and her CT scan had shown up a PA area consistent with a diagnosis of chronic PAP. The preop also shows highly sclerosed canals which decreased the chances of success with this tooth; However, under the microscope points of negotiation were explored and 2 canals located. The canals initially had to be instrumented with a 6k file and plenty of EDTA. Eventually we managed to prepare the tooth up to a WoG primary (25/07) while respecting the anatomy. With converging roots contemporary thinking is that we treat the main canal to length and then the 2nd canal to the point of convergence. Tricky tooth and not as simple as it first appears.

One of Schilder's principles for biomechanical preparation is that we should endeavour to respect the original anatomy of the root canal and that the prepared canal should contain the original canal.

This case had a slight double curvature on the distal root which would almost certainly be straightened out by old austenitic NiTi files. More modern Martensitic NiTi files in good hands really help maintain the original anatomy of the tooth.

This was a 56 year old female with asymptomatic periapical periodontitis. I placed a composite core and now she is all ready for a crown to finish a nice and tidy case.

Lower 6's have 4 canals 50% of the time. It is also true that approximately 30% of under 30's have a mid mesial canal. It is important to look for these canals to ensure none are left untreated and become a cause for failure.

MB2's in upper 6's are present more often than not with papers suggesting anywhere between 60-90% of these teeth having these canals. They are commonly missed and can be a cause for primary endodontic failure. This was a tricky case with a very narrow MB2. A proglider from dentsply was used to navigate this canal.

I really find diagnostic radiographs an essential part of the overall treatment. This lady attended with a lump between her UL6 and UL7. A thorough history and examination along with endofrost lead me to believe the UL7 was th cause of the problem. A confirmatory PA was taken with a GP point in the sinus.

It is often advisable to perform treatment with a draining sinus over 2 visits with an inter appointment dressing of CaOH. This was done in this case and on the return appointment the sinus had healed and treatment was completed.

Endo through crowns is always very challenging. Firstly theres the inherent loss of external anatomy which can cause a loss of bearings and of course there is the dread of working in a small dark hole where nothing is really visible.

This case was such a case. Working through metal ceramic crowns with poor light can be dangerous, microscopy with axial light is an essential in a case like this.
