Exactly one year ago today - 𝗧𝗛𝗥𝗢𝗪 🔙

12-months ago, our Practice Principal, Dr. Nida Kamal, donated 14 inches of her hair to the The Little Princess Trust, helping children experiencing hair loss due to cancer treatment and other medical conditions.

Together, with the incredible generosity of our patients, colleagues, friends and family, we also raised £2,406 to support this amazing charity 🎗️

Thank you again to everyone who donated, supported and helped make such a meaningful difference. After exactly one year, it is the perfect opportunity to take a look back at how it all went 💜

Sometimes the hardest part of retreatment isn't finding the problem... it's deciding what to do first. 🤔🦷

This LL6 definitely kept me on my toes.

What started as a routine retreatment quickly turned into:
👉 a previously untreated second distal canal
👉 a failed furcal perforation repair that was much larger than the radiograph had suggested

The first visit was all about removing the previous root filling, locating and preparing all four canals (including the missed distal canal) before dressing with calcium hydroxide.

By the second visit the canals were dry and ready to fill... but one question remained:

Repair the perforation first... or complete the root filling first?

For this case, I chose to obturate the canals first, protecting the canal orifices before carefully revising the perforation repair with bioceramic putty and completing the definitive coronal seal.

Every retreatment is different. Sometimes the challenge isn't the instrumentation—it's adapting your plan when the clinical picture doesn't match the radiograph.

One missed canal.
One failed perforation repair.
One very satisfying finish. ✨

Now we wait and let biology do the rest. 🤞

💚 My fingers after this one... absolute Hulk fingers! 💪😂

Some cases really make you earn every millimetre.
This LL6 was referred in pain and required treatment over two visits. The first appointment was all about getting the patient comfortable. The second... well, let's just say the mesial canals weren't giving anything away!

A million K-Flexo 10 files later (slight exaggeration... maybe 😅), patience finally won. Every millimetre in the mid and apical thirds had to be negotiated before patency was achieved.

The best part? Despite being incredibly nervous, my patient trusted the process and coped so well throughout. Those are the cases that make all the concentration (and the sore fingers!) worth it. 😊

🔬 Two-visit treatment: Single-tooth rubber dam isolation, 5.25% NaOCl throughout, zero lengths with Morita ZX, 20.04 prep, passive ultrasonic irrigation with EDTA 17% as penultimate rinse, warm vertical compaction (distal) + modified hydraulic condensation (mesials) with Edge Bioceramic Thermal Flow Sealer, followed by Clearfil Dual Cure composite core to finish. Et voila! ✨️💫👌😅

Endodontics is often a game of patience. Sometimes you don't need bigger files, you just need persistence...and endo fingers 👌😅

A fantastic root canal for my fantastic little patient! He was so brave and I'm so proud of him 🥰 This was secondary to trauma after playing sports, bless him.

Tx: 1st appointment was a quick open and dress as the poor kid was experiencing pain from the tooth. Usual local anaesthetic protocols, sectional isolation UL2-UR2, access through P aspect of tooth, pulp extirpated, copious NaOCl 5.25% throughout, canal dressed. Zero length with Morita Z, canal prepped to F5, apical gauge 65, canal dressed with Ultracal XS, ptfe tape and IRM. Patient booked back for completion of treatment.

Visit 2: Same protocols as above. NaOCl 5.25% throughout, EDTA 17% as penultimate rinse, passive ultrasonic irrigation, matched F5 GP cone, Edge Bioceramic Thermal Flow, warm vertical compaction, gp cut back to below CEJ, Clearfil dual cure composite core to finish-et voila!✨️💫

A wonderful 2 year review for my wonderful patient. His last root canal lasted 50 years and I promised him this one would do the same! No pressure 😅

The nitty gritty:
Tx was over two visits, under usual local anaesthetic and rubber dam principles. GP was removed with hedstroms, patency achieved with EDTA 17% and small handfiles. Prepped to F2, copious NaOCl 5.25% throughout, EDTA 17% as penultimate rinse with passive ultrasonic activation. Matched GP cone, PCS, warm vertical compaction, dual cure Clearfil composite, et voila! 💫✨️👌

37°C then. 37°C now. Five years apart 🌡️

The PPE photos are from 2021: full respirator, surgical gown, face shield, all in a non-air-conditioned surgery in the middle of a heatwave. I genuinely don't know how we got through those days, but we did.

Today, same temperature outside, completely different story. Just scrubs, scope, and so much gratitude for how far we've come.

A little PSA while we're at it: please stay hydrated out there today, whatever you're doing! 💧
