One of the more interesting things guided surgery teaches you over time is how much instinct can influence implant positioning.

When you review enough cases, certain patterns start becoming obvious.

Visibility, access, and hand position can all influence what feels “right” clinically during placement.

Guided surgery changes that. The planning stage becomes a rehearsal before the surgery even begins.

Over time, that changes how you approach implant placement clinically.

Guided surgery doesn’t remove judgment, but it does reduce the tendency to rely purely on subjective positioning during surgery.

A lot of these concepts, particularly around workflow control, planning logic, and reducing variability, are exactly why I wrote my book Guided Surgery: Making Implant Placement Simpler.

Complex restorative cases are never about one discipline alone.

This case involved a combination of:
✔️ Root canal treatment with endodontist David Rawson Caine
✔️ Soft tissue grafting
✔️ Ceramic bridgework, veneers, and crowns

The key in cases like this is coordination, making sure every stage supports the next, both biologically and aesthetically.

The best outcomes happen when surgery, endodontics, and restorative dentistry are all aligned from the start.

15 years later, and this is what stability looks like.

This patient had existing spaces at the LR5 and LL5 sites, which were restored with two implants placed back in 2011.

📸 She recently returned for a review, and both implants continue to look stable and healthy after 15 years in function.

The patient remains very happy with the result, which is always the goal.

It is interesting to see how well the deeply placed implant has performed, despite it likely being perceived at the time as being placed too deeply. This is something we see regularly in clinical practice, where implants that were positioned deeply continue to function successfully and remain stable over the long term.

Guided surgery is far more accessible now, with better software than ever before, clearer workflows, and more predictable planning tools.

Live Guided Surgery - Part 6

This stage focuses on implant placement and grafting.

The implant was placed with controlled torque, then finalised manually to ensure correct alignment with the planned position.

The jump gap was grafted and densely packed, supporting the site and helping maintain volume during healing.

This is where control matters, not just placing the implant, but managing the environment around it as you do so.

Looking back at a trip to Northern Wales.

The landscape genuinely reminded me of parts of New Zealand, particularly the roads and mountain scenery. Driving through some of these areas felt very familiar.

Always good to step away from the practice for a few days before getting back into surgery, planning, and teaching again.

Good scenery helps.

This is what careful planning and guided implant surgery can support over time.

This patient started treatment with me in 2020, presenting with difficulty chewing and a lack of posterior support.

The plan involved rebuilding the bite with implants at UR6, UR4, UL4, UL6, LL5 & LL7, alongside:
✅ Lateral sinus grafting (UL)
✅ Internal sinus lift (UR)

After a period of healing, we restored the case with screw-retained implant bridges across the posterior segments.

At his most recent review, everything looked stable:
✅ Healthy soft tissues
✅ Stable bone levels on X-ray
✅ Functional, comfortable bite

The result is a patient who can function properly again with teeth that are built to last.

Live Guided Surgery – Part 5

This stage focuses on the drilling protocol and site preparation.

A controlled, slow drilling sequence without irrigation was used, starting with the profile drill, followed by pilot and progressive drills up to the final size.

Bone was collected during the process using a bone trap, ready for later use.

This is where attention to detail matters.
Small steps help optimise the site before implant placement and support better outcomes.
