Live Guided Surgery - Part 6

This stage focuses on implant placement and grafting.

The implant was placed with controlled torque, then finalised manually to ensure correct alignment with the planned position.

The jump gap was grafted and densely packed, supporting the site and helping maintain volume during healing.

This is where control matters, not just placing the implant, but managing the environment around it as you do so.

Looking back at a trip to Northern Wales.

The landscape genuinely reminded me of parts of New Zealand, particularly the roads and mountain scenery. Driving through some of these areas felt very familiar.

Always good to step away from the practice for a few days before getting back into surgery, planning, and teaching again.

Good scenery helps.

This is what careful planning and guided implant surgery can support over time.

This patient started treatment with me in 2020, presenting with difficulty chewing and a lack of posterior support.

The plan involved rebuilding the bite with implants at UR6, UR4, UL4, UL6, LL5 & LL7, alongside:
โœ… Lateral sinus grafting (UL)
โœ… Internal sinus lift (UR)

After a period of healing, we restored the case with screw-retained implant bridges across the posterior segments.

At his most recent review, everything looked stable:
โœ… Healthy soft tissues
โœ… Stable bone levels on X-ray
โœ… Functional, comfortable bite

The result is a patient who can function properly again with teeth that are built to last.

Live Guided Surgery โ€“ Part 5

This stage focuses on the drilling protocol and site preparation.

A controlled, slow drilling sequence without irrigation was used, starting with the profile drill, followed by pilot and progressive drills up to the final size.

Bone was collected during the process using a bone trap, ready for later use.

This is where attention to detail matters.
Small steps help optimise the site before implant placement and support better outcomes.

Emergency anterior cases are always about balancing immediacy with long-term predictability.

This patient attended in 2022, having fractured his UR2 tooth.

The options discussed were:
โ€“ Root canal treatment + post + crown
โ€“ Extraction and implant replacement

He opted for the implant solution.

The surgical photos show the fabrication of the temporary implant crown, which plays an important role in supporting the soft tissue and guiding healing in the aesthetic zone.

๐Ÿ“ธ The final images show the case at review two years later, with stable tissue and a natural-looking result.

After placing thousands of implants, one thing remains true: successful anterior implants rely on careful planning, tissue management, and controlled ex*****on from day one.

From single tooth to full arch, this is how cases evolve.

This patient first had the UR1 extracted and replaced with an implant in 2019.

Following this, he wanted to address other historic missing teeth and improve the overall colour and consistency of his restorations.

We subsequently placed four additional implants, two upper left and two upper right, and restored the upper arch with a temporary implant/crown bridge during healing.

Once integrated, the case was completed with:
โ€“ Two 3-unit implant-retained bridges
โ€“ A definitive UR1 implant crown
โ€“ Porcelain restorations on the remaining natural teeth

๐Ÿ“ธ The last images show the patient at review in 2024, with stable tissues, healthy bone levels, and restorations continuing to function well.
