This old silver filling was hiding decay underneath.

For this lower right molar, I removed the old amalgam filling and cleaned out the decay without exposing the nerve.

I then placed a fresh composite core before restoring the tooth with a ceramic restoration.

The aim was to restore the tooth while preserving as much of the remaining tooth structure as possible.

Not every tooth with a large filling needs a crown straight away. 🦷

But when a tooth has been heavily restored, there comes a point where replacing the filling alone may no longer be the most predictable option.

In this case, the existing restoration was replaced with a ceramic crown to help protect the remaining tooth structure and restore long-term function.

The aim is always to preserve the tooth while creating a restoration that feels comfortable, functions naturally and blends in with the surrounding teeth.

One of my favourite cases of the year so far ✨

We started with Invisalign to gently align the teeth and close those unwanted gaps, creating a more balanced and natural smile. Once everything was in the ideal position, we enhanced the colour with professional whitening for a brighter, fresher look.

To finish, we used composite bonding to carefully restore the tooth structure that had been worn down from grinding — rebuilding shape, strength, and symmetry while keeping everything looking natural.

A great example of how combining treatments can completely transform a smile while still keeping it subtle and personalised.

Upper right first and second molar direct composite fillings 👌🏽

Secondary decay around both, dentine pins in second molar…I hate pins! Good adhesion under rubber dam is satisfactory!
