Decortication 🕳
Research shows thats Statistically its not significant but what about clinical significance?
Clinically would it increase mechanical retention of an onlay graft or is the main benefit just increasing blood supply?!
If you are doing it do you go for a bigger perforation or smaller, research again shows that at 12 weeks smaller perforations and larger show the exact same results and similar histological appearance!
What’s your go to bur for this?!

Tissue thickening!
Posterior mandible where the tissue is notoriously thinner, I like to throw in something to bulk it up

This case involved implant placement amd simultaneous use of Puros Dermis Allograft Tissue Matrix from
3 months later beautifully thickened and mature soft tissue.

Window on latch!
With thicker sinus walls I’m quite happy leaving this little window in situ, forms the new floor of the sinus quite nicely (yellow arrow on xray).

With thinner sinus walls I’m more inclined to remove it just because thinner windows have a tendency to be sharper and the last thing we want is for this to tear the membrane after you’ve spent an age carefully lifting it 🤦🏾‍♂️
