I've been in endodontics long enough to know that the cases that teach you the most are rarely the complicated ones.

It's the simple case that turns out not to be. The tooth that looks fine on the radiograph until it isn't. The patient who mentioned something in passing at the end of the appointment that turned out to be the most important thing they said.

Endodontics rewards attention. Always has. 🦷

The quality of obturation matters more than it sometimes gets credit for.

Inadequate fill, voids, or a poor coronal seal don't always cause immediate problems, but they create the conditions for reinfection that can manifest months or years later, often in ways that are harder to manage than the original case would have been.

Root canal treatment is only as good as the seal it achieves.

It's not always easy for patients to identify the source of dental pain.

Referred pain from a tooth with irreversible pulpitis can present convincingly in adjacent teeth, the opposing arch, the ear, or the jaw, leading patients and clinicians alike toward the wrong diagnosis. Pulp testing multiple teeth, taking targeted radiographs, and applying careful clinical reasoning is more reliable than guessing together at where the pain is coming from.

Sometimes it's worth talking through a case with a specialist.

Vertical root fractures are one of the most challenging diagnoses in general dental practice, and therefore one of the most commonly missed.

Symptoms are often vague:

- Mild to severe pain when chewing
- Recurring fistula on the gums, or localised swelling
- Isolated periodontal defect

But the absence of a clear radiographic sign doesn't mean the tooth is fine.

If something doesn't add up, it's always worth considering a referral.
