Cases like this require careful staging to ensure long-term stability. Here is the exact clinical and digital workflow used to restore function and aesthetics:
Phase 1: Initial deprogramming using a Lucia Jig to capture a reliable centric relation.
Phase 2: Trial period with temporary restorations fully designed in Exocad, to assess TMJ tolerance and patient adaptation to the new vertical dimension.
Phase 3: Definitive restorations utilizing layered e.max for the ideal balance of strength and optical properties.
Phase 4: Long-term protection via a Michigan Splint. This was designed natively in the iTero Design Suite and 3D printed inhouse on the SprintRay Pro 2 using NightGuard Flex resin.

Treatment tailored to a Bolton discrepancy.

Rather than closing all the upper spaces orthodontically, with potential challenges for long-term retention, I used the available space to address the patient’s small, worn teeth.

We redistributed the spaces, corrected the anterior crossbite and restored tooth shape and colour to the patient’s preferences. Throughout, we preserved the subtle asymmetry that gives their smile its natural character.

We finished the case with our new hybrid 3D-printed Michigan splint for the upper teeth and a Vivera retainer for the lower teeth.

Uneven spacing, an edge-to-edge bite and a crossbite. In this case, a tooth size discrepancy was responsible for the gaps.

Invisalign alone could have closed the spaces. Veneers without Invisalign were also possible. But neither approach alone would have delivered the stable bite, balanced proportions and smile symmetry we wanted.

Closing all the spaces orthodontically would have made achieving a Class I bite and maintaining the result more challenging.

After discussing these considerations, the patient understood why combining Invisalign with veneers would address both tooth position and tooth size.

Together, we agreed on an Invisalign restorative approach.
