This is a case of failed and broken teeth ULQ. The teeth were extracted. 2 implants were placed immediately combined with and membrane to provide protection to the graft material. A final screw retained was fitted.

Socket preservation or alveolar ridge preservation is a procedure to reduce bone loss after tooth extraction. This is a case of upper central tooth in a patient with high smile line. A prime area for this procedure. Bone graft including PRF membrane and "stick bone" used to preserve the socket. An immediate resin retained bridge used to replace the extracted tooth.

A case of external cervical . Possible aetiology orthodontic treatment. Full thickness flap raised, area cleaned leaving large cavity but no exposure. was used to restore the lesion. Pt warned about possible need for should he develop symptoms and to expect some gum recession. Long term monitoring will be needed 😊

This patient was referred to me in 2008.
He presented with sign of sever periodontitis affecting tooth 13.
The treatment included tooth extraction followed by implant placement, GBR and FGG.
Unfortunately, I did not see the patient for 10 years!
Few days ago, he was referred again for RCT 15.
This allowed me to review the implant progress after 10 years of service.
The implant showed sign of stable bone level with minor mucosal recession.

I am a big fan of single visit RCT. However, one need to be confident that the root canals were cleaned throughly and dried before obturation. In cases of present pain,swelling, discharge and indeed pus in the pulp chamber it is better to have staged approach.

How best to diagnose peri-implantitis?
This case was referred to me last week. The patient reported tenderness in the region of 21 implant. The implant was placed 10 years ago with no reported complications.cally, no clear swelling was noted. The recorded X-ray showed little change from the one recorded a year ago.
Interestingly, the recorded BPE score from the referring dentist was 2.

My approach is to perform combinations of tests. This is resulted in the following:
- Close visual inspection of the cervical margins of inflamed mucosa showed puffy margins.
- The palpation the area produced tenderness
- Massaging the mucosa from the base of the implant towards the cervical tip showed some discharge with bleeding.
- Mobility assessment resulted in micro-mobility.
- The tapping test produced dull noise which is not consistent with integrated implants.
- However, the best reliable test in my opinion was probing under LA. The result was self-explanatory in the attached video.
Unfortunately, this implant prognosis was poor, and it will be removed.
