Class IV composite. Shape, primary anatomy, secondary anatomy and tertiary anatomy. All these must be accurate and appropriate before we even get to ‘the shade’…and then it’s not just the colour, it’s the opacity and relative translucency and spatial arrangements of layers. Not needed for every case but if you understand them, then you have a fighting chance of nailing every case. Direct composite restoration case from last week ✨

Cases like this are always a reminder that even when we’re happy with the outcome, there’s always another level of refinement we can aim for. In this case, the goal was to recreate the natural character of the tooth as closely as possible — particularly the intense translucency, which needed careful layering and management to achieve a lifelike result.

This was such a rewarding case to work on, and it highlights just how artistic and technique-sensitive direct composite dentistry can be.

These cases are tough! A unilateral dark tooth paired with an open gingival embrasure. Not the easiest combo.

Success here isn’t just about placing composite… it’s about strategy:

🚨 When crowns don’t fit properly, your gums will tell you.
This patient came to me with poorly fitting, unaesthetic crowns that were causing inflammation and bleeding. Not only did they look unnatural, they were also compromising the health of the surrounding gums.
So we went back to the fundamentals.
First, I adjusted the gum levels to create better shapes and created well-fitting temporary crowns to allow the tissues to heal properly.
Next, an implant was placed to replace the missing upper left lateral incisor.
Finally, we restored the teeth with custom zirconia crowns designed to mimic the look and behaviour of natural teeth.

Injection moulded composite restorations to restore worn teeth in the maxilla. One of my main uses of IMT these days is probably this. It’s a fantastic tool to transfer a new occlusal scheme into the mouth using the copy and paste system. This patient underwent orthodontic alignment first using Invisalign before restoration of the teeth.

Diastema closure with direct composite. There are many ways to tackle these cases. My preference is using bioclear matrices. I find it gives a superb, ultra-cleansable emergence profile. There are several steps along the way to make things predictable: good isolation, appropriate matrix selection, and making sure you don’t have a cant!
