Really difficult and rewarding case. This patient had suffered facial trauma resulting in 3 fractured anterior teeth (I seem to be just treating 3 unit cases at the moment). The UR1 had a deep fracture subgingivally on the palatal aspect. Following root canal treatment I performed deep margin elevation before preparing the tooth for a crown. I also placed veneers on the remaining fractured teeth. Smile restored. A very happy patient and dentist.

I hear this concern often.

But a well-designed crown should not draw attention to itself.

This case involved replacing a single old metal-ceramic crown with a carefully designed zirconia crown, created to complement the natural form, colour, value, translucency and surface texture of the surrounding teeth.

The aim was not to create a “perfect” tooth.

It was to create a restoration that looked natural and belonged within the smile.

This lovely patient came to me specifically to have veneers placed on her teeth. She was fed up with the mismatched colour of her smile. Previous trauma had caused 3 of her front teeth ( UR2, UL1, UL2) to need root canal therapy and, whilst this treatment had saved her teeth, they had subsequently gone dark in colour. She had undergone internal bleaching in the past which had not been that successful and now wanted to go down the veneer route. You can also see that she had a number of mismatched and leaking fillings on her front teeth which were adding to the colour issues. Veneers might be a good option here but careful examination of her teeth showed that, because of the previous treatment, there was little natural tooth structure remaining. If I wanted to mask the dark colour and want the final veneers not be overly bulky, I would have to remove more natural tooth structure and increase the risk of potential fracture. I will always try to start with the most minimally invasive options for patients, and even though she had tried internal whitening before she trusted me enough to give it another go. This treatment does rely on good patient compliance and sometimes a patient patient. She knew that we were trying to get her the smile she wanted in the most tooth friendly way possible and was patient enough to work through the procedure of first internally bleaching her UL1 and UL2 before vital bleaching of her other teeth. The old composite restorations were then replaced to match the new lighter colour of the teeth. A brighter more harmonious smile achieved and a happy and grateful patient.

This patient wanted a fixed, natural-looking solution for his missing teeth—while preserving the healthy teeth he already had.

Not every smile transformation is straightforward—and that’s exactly where comprehensive treatment planning makes all the difference.

This case presented multiple aesthetic and restorative challenges, including missing teeth, a midline diastema (gap between the front teeth), and a canted, off-centre dental midline.
