Transforming a Complex Smile: 17-Year-Old with an Open Bite
I’m excited to share a complex case currently in treatment—a 17-year-old with an anterior open bite, caused by a downward/backward jaw growth pattern, narrow upper jaw, and a history of thumb-sucking. Cases like this require a meticulous, customized approach to achieve functional and aesthetic excellence.
The Challenge:
Correcting this bite means balancing jaw alignment, managing growth-related changes, and ensuring the smile looks natural. Every step demands precision, planning, and expertise to maximize predictability in a case this intricate.
The Plan:
1️⃣ TAD-Based Expander: Instead of relying on traditional expanders that are unreliable at this age due to suture fusion, I utilized a TAD-supported device for stable and controlled upper jaw expansion.
2️⃣ Posterior Intrusion: By incorporating a TPA/Intrusion Bar, I’m gently intruding the back teeth to allow the lower jaw to auto-rotate forward—an advanced method to naturally improve the bite.
3️⃣ Braces + Aligners: Fixed braces uprighted the incisors to set the foundation for the next phase. We’ve now transitioned to aligner therapy for exceptional vertical control and precision.
