How curved is too curved? Swipe ➡️

Externally referred upper molar with gross caries. The tooth had to be made restorable first: caries removed, the deep margin elevated and a bonded core placed, giving a sealed chamber to work in.

The mesial root had a severe curve 😳! It was shaped with the new Reciproc Minima to hold the original canal path, then obturated to length in 1 visit McGrath Dental

⭐️ Sclerosed UL7, single visit ⭐️

The canals were heavily calcified and barely visible on the pre-op.

• Crown removed for improved access and visibility
• Canals located and negotiated with 7.8 magnification
• Shaped with ProTaper Gold
• Irrigated with 3% NaOCl, then 17% EDTA, then a final 3% NaOCl rinse, all with manual dynamic agitation using the master GP cone
• Sealer placed with a Viscotip and the vapour lock broken with a clean size 10 hand file
• Master cone buttered with sealer, rotated as it was taken to length, then condensed with Buchanan pluggers
• Core rebuilt with SDR and Venus
• Temporary crown placed, and the patient returned crown-ready

You can see sealer filling the extra apical anatomy. This is what happens when irrigation and obturation reach beyond what files can shape.

Calcified molars can be tricky; perforations, ledging and missed canals commonly. When the pre-op looks like this, early referral gives the tooth its best chance.

Referring dentists: your patient always comes back to you, crown-ready, and you keep the crown fee. You’ll get a written report and radiographs within 7 days.

Advised extraction. Retreated instead. 🦷

This patient had been advised to have the tooth extracted because the lesion was so large, and came to me for a second opinion. I agreed extraction was the most predictable option, and put the chance of saving the tooth at around 50/50. The patient chose to try.

I usually complete root canals in one visit. This one needed two:

1️⃣ Remove the old root filling, re-prepare, disinfect, then a calcium hydroxide dressing for 3 months
2️⃣ Further disinfection and obturation

At 6 months (3 months of calcium hydroxide dressing, then 3 months after obturation), the patient is symptom free, the tooth is clinically sound, and the lesion is around 40% smaller with good bone infill. That’s excellent early healing for a lesion this size.

A large lesion isn’t automatically a lost cause. With an honest, consented prognosis, retreatment can work.

⭐️ Invisalign Smile Makeover ⭐️

Things we needed to correct-

❌ Upper lateral incisors sticking out
❌ Deep bite
❌ Upper central teeth tipped backwards
❌ Lower front teeth being worn down by the bite

Straightened in 11 months with Invisalign. No teeth removed.

The aim wasn’t just straight teeth. Fixing this type of bite can leave the top teeth sticking out too far if no teeth are removed, so this was carefully planned to avoid that.

🦷 3 teeth on the line. 1 visit to save them. 🙌🏻

This patient was referred in by another practice. Their UR2 needed root canal treatment, but it was
holding a 3-unit bridge: UR1 missing, with UR2 and UR3 joined as abutments.

One possible route would be to cut the bridge off, treat the tooth, then pay for a brand new bridge. That means more patient cost and more appointments.

Instead we went through the bridge. With magnification and a small, precise ninja access 🥷 , we kept as much of the bridge as possible and completed the root canal in a single visit. The bridge stayed in place, and the patient left with the same smile they came in with.

Straighter, brighter, and a bite that actually meets ✨

This patient wanted to straighten the crowded front teeth and improve the bite. During treatment, whilst correcting the front we ended up with teeth at the side not fully meeting when they bit together. Aligners alone don’t always fix that, so we added small tooth-coloured buttons and elastics to gently guide the upper and lower teeth together.

Straight teeth are only half the job. A good result needs a bite that works too.

We finished with professional whitening.

People assume nervous patients are frightened of pain. Mostly they are not.

They are frightened of lying back with their mouth open while something happens that they cannot see, cannot ask about, cannot control and cannot stop.

So I tell you what I am doing before I do it. Every step.

And if you want to stop, you raise your hand and we stop. Not after this bit, straight away. You are involved and have control.
