This week’s challenge ! UL4 referred to me by a very competent colleague - a great shout on their part - pulp chamber just “de-roofed” prior to seeing me to help manage acute symptoms .. this made it much, much easier for me to access the canals conservatively and carefully !
Prepped with 06/08/10/15 then Waveone Gold .. dressed for a few weeks as I couldn’t get a completely dry field and subsequently obturated with WVC & AH plus .. ready for core and onlay .. UL5 under r/v by referring GDP

This 73 year old gentleman presented with no significant medical history and a buccal sinus associated with a (strategically important) lone standing molar with a relatively shallow restoration .. I accessed it thinking there was probably a vertical fracture .. nothing obvious and found to my surprise there was some vital tissue remaining despite negative readings from cold testing and EPT.. so having committed myself and cleaning and shaping it with Protaper Gold .. I took a GP trace rad (post calcium hydroxide dressing).. and gave it a few weeks. Hey presto the sinus has healed and I’ve obturated now .. let’s see what happens with the periradicular tissues in time 🙏🏻

This is a recent case referred to me by a very dear colleague.. a stainless steel k-file which separated inexplicably during extirpation .. a high stress situation with a patient in pain - these things can happen to the very best of us.

The patient was informed and carefully managed and fortunately following a long and full discussion with the patient, we were able to remove the instrument with some time and patience !

The apical third of the canal was very fine and needed some careful preparation alongside quite a bit of coronal flaring by hand to ensure that the same thing did not happen again.

The patient, referring dentist and I were absolutely delighted with the outcome !

At The Sandstone Dental Practice we are always delighted to help colleagues with complex cases like this one !

Is it any wonder these MB2 canals are missed when working without microscopy ... ?! These two are so close together you would never see or feel them without good magnification ! I thought there may even be a third MB at one point due to the developmental lines !!

Even with the scope it was difficult to instrument without the file slipping into the wrong one and of course to obturate too!

I completed this case last week - the history is an asymptomatic tooth which was subject to trauma many years ago - the resorption was picked up as an incidental finding on a CT undertaken for implant planning bilaterally in the posterior maxilla. Thankfully the conscientious treating clinician checked the whole scan & fortunately the resorption had not breached the walls of the root to communicate with the periodontium.

The treatment was undertaken (via the existing crown as Pt was very keen not to have to replace unless absolutely necessary) over 2 visits. The obturation completed with warm GP and bioceramic sealer 😊
