Sunday - seriously what am I working a Sunday for with everything else going on right now

You may see me doing a lot of endo, but I’m actually interested in restorative as a whole, but how can you pursue restorative without a strong grounding in the things you are gonna slap your crowns and composites on top of?

The same goes for perio, go through the hard yards for the shiny fun bits once you finish

Either that or get yourself into a great practice with people who will deal with the bits you don’t like in a collaborative effort

Back to this case, we are hoping to rehabilitate this mouth with crown and bridge work, the patient has made great strides in oral hygiene already and we are nearing the end of the endo perio stage and steaming ahead to the aesthetic work soon

Endo post cores in the ur2, ul2 and lr5 and an endo completed in the ur1, a couple more left now (coming soon)

All completed using my usual equipment (see below) but .medical are running a competition you should check out!

Joint case completed over 3 visits with

Diagnosis: PN/AAA

Cbct showed loss of buccal plate and a palatal sinus tract was located

Pathology spanned ur2-ur4 but luckily only the lateral was involved

Intra visit CaOH led to a complete resolution of symptoms and the healing of the sinus tract

We now enter a review period to assess healing radiographically in 6 months

If we dont see any resolution a surgical approach by at may be required

Premolars day 2/2

This one was another 2 rooted premolar, secondary caries under previous fillings (preop is actually an old film 4y ago) we had an up to date bitewing showing caries and i didnt feel the need to to irradiate for a new periapical with that view available (alara)

The first molar has healed significantly since the primary endo but we can see there is still a lesion so the pt is considering options atm

A different kind of endo today, and its a case of problem solving and not accepting a suboptimal outcome

Everything going well until obturation but the gp cone unseated or folded during the process and i ended up having to go back and redo.

The final looks good to me and im pleased with the seal especially distally

Tell me what you struggle with in endo and I’ll try arrange a live session with one of my endo geek friends to unpick it for you 💪🦷

This tooth tested me bigtime, the deep margin took an additional half hour to manage, endo itself not too much of a headache.

My takeaway for those of you who struggle with endo : set up the tooth for endo before you pick up a file, the endo might still be tricky but you need to give yourself the best chance to get the job done.

What does that mean practically?

🦷 Remove previous restorative
🦷 rewall where necessary to allow for a pool of irrigant and clamp stability
🦷 make sure you have good preop radiology (how are there people accessing a tooth without periapicals still 🤦🏾‍♂️)
