are also available. We also now offer Anti Wrinkle Treatments and Zoom Tooth Whitening. We are always keen to improve the service we provide and appreciate any suggestions you may have to acheive a higher standard of patient care. Appointments:
On your first visit to the practice you will sign an NHS form. This will be used as evidence of your attendance and registration with the practice. You may be asked to sign another form if you see a different dentist, this is necessary to conform with NHS regulations. You will also be asked to complete a medical history form. It would be useful if you could bring with you a list of any medications you may be taking along with your doctors contact details. Emergencies:
Acute emergencies will be seen within 24 hours (weekdays). After 5.30pm weekdays and at weekends, emergencies will be dealt with by "Fife On Call Service". This number is available on the practice answering machine after hours. If you require an emergency appointment please contact the practice between 8.30- 10 am. Cancellations:
24 hours notice must be given for all cancellations. Failure to do so or non attendance will result in a charge for wasted clinical time. Reception will be unable to arrange further appointments until the account is fully cleared. Payment for dental treatment:
NHS dental examinations are free of charge. Following examination you will receive an estimate of the cost of the treatment you need to have. Please ask reception if you do not receive one. Patients will be required to pay for treatment they have received on each visit to the practice. Exemptions:
Some patients may be partially or fully exempt from paying NHS charges. These include
Patients under 18 years old
Patients between 18 and 19 and in full time education
Patients who are pregnant
Patients who have had a baby in the last 12 months
Patients receiving certain government benefits
It is a legal requirement that we see proof of your exemption from payment.
