Last case of the week- separated instrument removal from MB canal of UL6.

UL6 non surgical retreatment completed back in 2022. Complete healing monitored over time.
UL7 has been vital and asymptomatic up until 2024, and then removed (external resorption caused by the impacted UL8).
Now the UL8 is attacking my UL6…

Trauma case from today. Happened 4 weeks ago. Pt c/o discomfort from her UR1 and lip adjacent to it.
UR2 had a post retained crown, which fractured, now most probably not restorable.
PA radiographs shows possibly external resorption apically on the UR1. But! Have a look at the cervical area of the UR1. Can you see an unusual radiopacity? I took a CBCT scan and my fear was confirmed. Part of the porcelain crown from the UR2 is buried in the patient’s lip!
Important point in any trauma case involving fractured teeth- always check soft tissue, especially lips, for the broken pieces.

Would you have done multiple visits for this one?
Large periapical lesion healing after only 3 months! Single visit.
Instrumentation-hyflex EDM up to 50/03.
Irrigation- hypo 5.25%, EDTA 17%, CHX 2%, PUI.
Obturation- Totalfil bc sealer, GP, WVC.

UR2 presented with a palatal groove, and 4 mm perio probing. Tooth deemed necrotic based upon radiographic appearance and lack of response to cold tests.
Rct performed over 2 visits.
I cannot remember ever seeing a lateral canal located that coronal. It is most probably associated with this groove.

Went fishing for some guttapercha.

Separated rotary instrument removal from mesial system of LR7. Lots of fine ultrasonic activation, high magnification and patience. Retreatment case, the crown will be replaced in due time.
