Oh YEAH …he’s back ..! (And Usher 😎)

Case 1: fractured file in a lower molar, US ACTEON ET25 and platform approach to remove the fragment ‘MI’ style. VS 17/04, F1/F2.

Case 2: M-D coronal fracture. Caught in time not in the pulp floor! 🙏🏽 chronic Irr pulpitis-> pulpal calcification, challenging 🙈, but managed to locate the MB2. EDTA solution, #8 KFlex, S1, VS17/04, F1, F2 (pal).
SDR/Spectra. Composite layered core and overlay.
Back to referring Dr for the crown.

Case 3: Review of a case that I saw Aug 2023: CAA, necrosis UL6, one visit (VS, ProTaperGold), composite core. Complete healing, discharged back to referring Dr for the crown.

Referral endo practice brings with it some daily challenging cases:
- some are outright tricky form the start 🤔
- some initially appear straightforward but turn out to be a pain 🤦🏽‍♂️
- others start off a pain but turn out to be pleasantly good 😁

3 cases that I saw yesterday that encompass all of these:

CASE 1: UR6
Symp AP after pulp necrosis, crowned. Really apically calcified and unable to achieve patency in either buccal roots.
Plenty of EDTA and US activation and, low and behold, apical clearing! 🤗

CASE 2: Re-RCT UR1
General dentist had started the reRCT a few months ago after a Symp AP and swelling, but unable to remove the apical gp.
I suspected a ledge, which there was! 🙄
Solvent used to soften and renegotiate the apical gp, pe*****te, remove, and then ‘flatten’ the iatrogenic buccal ledge. 1 visit RCT/RCF 👍🏽
(UR2 being extracted soon)

CASE 3: LL 2nd molar
Irr pulpitis -> pulp necrosis
Tricky to negotiate and partially calcified 3 canals and curved apical anatomies 😯

All cases treated with VSFlexi gold and Protaper Ultimate.
