🚨 Pre-Wedding Rescue Mission! 💍
A patient came in for whitening, but our exam revealed a massive silent cavity on the LR5 distal and another creeping into the LR6 mesial. Caught just in time before the honeymoon!
Workflow for this back-to-back Class II save 👇
🚀 1. Efficient Access: One large bur for fast gross reduction, saving time for the bonding protocol.
🩸 2. Tissue Management: Thermacut bur for a localised gingivectomy. The wedge must seat below the deep margin.
🪵 3. Pre-Wedging: Protects papilla and creates separation.
🎯 4. Peripheral Seal Zone: Caries detector dye is useful. Clean the outer 2–3mm to establish a reliable seal, then safely perform selective caries removal deeper down—leaving remineralisable dentine over the pulp.
🪥 5. Clean Substrates: Interproximal strip to remove plaque. You cannot bond to plaque.
🌪️ 6. Air Abrasion: Aluminium oxide to clean and create an ideal micro-retentive foundation.
👯♀️ 7. Dual Matrixing: Place both sectional matrices so one filling doesn’t invade the other’s space. LR5 was restored first (Tor VM saddle matrix + spring clip).
🔄 8. Molar: Removed the saddle matrix and adapted a Garrison Strata-G V-ring to restore the LR6 mesial.
✨ 9. Finishing: 12d blade for marginal flash, Komet Q-finishers, Soflex discs, and ASAP polishers for a plaque-unfriendly result.
